ABOUT DIBAC HOME SUPPORT
Dibac Home Support is a licensed Temporary Help Agency providing personal care, household support, postpartum care, and in-home childcare to families and facilities across the Kitchener-Waterloo-Cambridge region. THE ROLE
Dibac Home Support is seeking a Registered Nurse to join our team as Director of Care — our clinical lead for home care client assessments, care planning, and frontline staff supervision. This is a meaningful, flexible role for an RN who wants to use their clinical expertise in a community setting, provide real leadership to PSWs and care workers, and help families get the right support at home. You will be the clinical backbone of our quality care program.
WHAT YOU WILL DO
· Conduct clinical assessments of home care clients — determining care needs, functional status, and risk factors
· Develop and review individualized service plans in collaboration with clients, families, and the Dibac coordinating team
· Provide clinical supervision and mentorship to frontline PSWs and postpartum care workers
· Conduct supervisory home visits to observe care quality and support worker performance
· Act as the clinical resource for frontline staff — guiding escalation decisions and supporting complex client situations
· Review incident reports and contribute to investigation, corrective action, and documentation
· Support PSW training and competency evaluation from a clinical perspective
· Contribute to Dibac's quality assurance program and policy reviews
This position is open to Registered Nurses (RNs) in good standing with the College of Nurses of Ontario. RPNs with demonstrated clinical leadership experience may be considered.
WHAT WE ARE LOOKING FOR
· Current RN registration with the College of Nurses of Ontario — in good standing
· Minimum 3 years of clinical nursing experience — community health, home care, or a related setting preferred
· Experience in clinical assessment and care planning
· Experience supervising or mentoring PSWs or other unregulated care providers — an asset
· Strong knowledge of Ontario's home care and personal support legislative framework
· Valid driver's licence and reliable vehicle for client and supervisory visits across KWC
· Legally eligible to work in Canada
