Role: Primary Care Nurse Practitioner – Youth Wellness (YWHO)

High-Level Overview for Candidates
TAPP Workforce Solutions is recruiting a Primary Care Nurse Practitioner on behalf of a community health organization serving diverse populations in Scarborough, in partnership with a youth-focused mental health agency.
This role is embedded within the Youth Wellness Hubs Ontario (YWHO) model, providing low-barrier, youth-centred care for individuals aged 12–25. The focus is on mental health, substance use, and holistic primary care within a collaborative, interdisciplinary environment. Some evenings and occasional weekends are required.
Key Responsibilities
  • Provide episodic primary care to youth (ages 12–25)
  • Manage acute and stable chronic conditions
  • Integrate mental health and substance use care into treatment planning
  • Order diagnostics and prescribe medications within scope
  • Apply trauma-informed, harm reduction, and culturally responsive care
  • Collaborate with an interdisciplinary team (therapists, peer support, etc.)
  • Participate in care planning and case discussions
  • Support health education and system navigation
  • Contribute to quality improvement and program development
  • Provide mentorship and clinical leadership
Requirements
Education
  • Master's degree in Nursing with Primary Health Care Nurse Practitioner certification
Certifications
  • Registered in good standing with the College of Nurses of Ontario (Extended Class)
Experience
  • Experience working with diverse and underserved populations
  • Experience in community health or primary care settings (preferred)
  • Experience with youth, mental health, and/or substance use (preferred)
Technical Skills
  • Strong clinical assessment and decision-making skills
  • Excellent communication and collaboration skills
  • Commitment to equity, diversity, and culturally responsive care
