HOME AND COMMUNITY CARE NURSE (FULL-TIME)

WAGE: $32.00-$35.00/HOURLY

The Home and Community Care Nurse provides nursing care and health services to clients in their homes and in the community. This role supports individuals to live safely and independently while promoting overall health and well-being.

DUTIES AND RESPONSIBILITIES:

• Provide in-home nursing care including assessments, wound care, medication administration, and chronic disease management.
• Develop, monitor, and update individualized care plans with clients and families.
• Educate clients on health promotion, disease prevention, and self-care.
• Coordinate services with other health programs and external providers.
• Support hospital discharge planning and continuity of care.
• Advocate for clients to access health and social supports.
• Maintain accurate, confidential records and ensure compliance with policies.
• Participate in team meetings and collaborate with health professionals.

QUALIFICATIONS:

• Current registration as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in Ontario.
• Experience in home care, community health, or acute care nursing is an asset.
• Knowledge of chronic disease management, palliative care, and health education.
• Strong organizational, communication, and interpersonal skills.
• Cultural sensitivity and understanding of Indigenous health and community values.

ADDITIONAL REQUIREMENTS:

• Current CPR and First Aid certification.
• Valid drivers license and reliable transportation.
• Criminal Record Check with Vulnerable Sector Search.

WORKING CONDITIONS:

• Travel within the community for home visits.
• Flexible schedule, occasional evening or weekend work to meet client needs.
