Position Description:
Reporting to theDirectorofIntegrated Care, theManagerof Integrated Care willleadand support the Director of Integrated Carewith executingengagementstrategieswith key partners toexplore innovative solutions and continuously improveclinical workflows.This position is for a creative leader with a passion foradvancing patient care anddigitalhealth; who is also an exceptional personskilled at motivating andengaging with others.It is a positionfor thosewho recognize the need to get the right thing done andpitchesin toaccomplishimportant goals.TheManagerof Integrated Care willwork as part ofthe Integrated Care teamtoact as a central support and subject matter expert for key collaborators such as OntarioHealthTeams (OHTs)totackle ongoing challengeswithin healthcare.
Key Roles and Responsibilities:
- Leadand supporteffortsacross the Integrated Care teamtoliaisewith key collaborators(e.g., OHTs, PCNs)andrepresentAmplify Careastrusted partners and experts in integrated care
- Collaborate and build consensus with external partners to support the development ofvirtually enabledmodels of care
- Convert partner-provided direction and information into strategically-soundideasandsuccessfully implemented deliverables
- Drive innovation by continuously taking initiative toidentifyimprovements in clinical workflows
- Work withthe Director of Integrated Careto execute activities and strategies in alignment withAmplify Caresmissionto be the leading, trusted, digital health partner for primary care and integrated patient care
- Anticipate, understand, and respond to the needs of internal and external partners to meet or exceed their expectations within organizational parameters
- Supportthe development ofefficient and suitable strategies to deliver integrated care initiatives
- Collaborate with Integrated Care team members to share knowledge and mine insights from emerging trends and prospective new integrated care solutions
- Regularly report updates tothe Director of Integrated Care
- Perform other duties asrequired
Experience, Skills and Qualifications:
- Minimum5years of relevant experience, includingleadershipofcross functional and multidisciplinary teamsandoversight of information systems and projectsin healthcare
- Post-secondary degree in Health Informatics, Information Technology, Business Administration, Leadershipor equivalent combination of relevant education and experience
- Change management experience and certification an asset
- Project Management Professional certification or equivalentanasset
- Proventrack recordof success inestablishingpartnerships withhealthcare organizations
- Knowledgeof emerging technology trends in healthcare
- Knowledge of health information systems, technology, and software
- Strong conflict resolution,facilitation,negotiationand mediation skills with the ability to build consensus across a diverse group of collaborators
- Exceptionalcommunication skills with the ability to effectively engage, collaborate, and build partnerships with keyinternal and externalcollaborators
- Demonstrated agility in learning andacclimating to unfamiliar business areas or technologies
- Prior experience working efficiently in a diverse and fluid working environment
- Ability to travel within Ontario as required
