Position Title: Director of Integrated Care
Work Hours / Status: 35 Hour Work Week - Full Time Permanent
Salary: $132,000 $142,596
Position Reports To: Chief Operations Officer

Summary Purpose of the Job
The Director of Integrated Care is a part of The Indigenous Primary Health Care Council (IPHCC)s senior leadership team and is responsible for providing consistent strategic and organizational support to teams by planning, managing, and evaluating activities and outcomes for the delivery of high-quality inter-professional health care programs and services for optimal individual and community health outcomes.
Key Duties/Responsibilities
- Provide strategic leadership and operational management for clinical and community programs and services.
- Plan, develop, and implement initiatives supporting Indigenous Health Models, well-being, and health equity.
- Collaborate with Program Managers to promote best practices and high-quality Indigenous and community care.
- Lead, monitor, and analyze continuous quality improvement and clinical initiatives, including oversight of Quality Improvement Plans.
- Handle complex member situations or crises requiring advanced training.
- Develop evaluation methods for programs and implement recommended improvements.
- Foster a collaborative, servant leadership environment, supervising and coaching direct reports, and ensuring effective delegation.
- Oversee recruitment, retention, performance management, and professional development of staff in partnership with Human Resources.
- Develop and maintain community partnerships to enhance service capacity and accessibility for IPHCC members and priority populations.
- Participate in Accreditation, board meetings, annual budget development, and business continuity planning.
- Advocate for member sites and communities, provide care delivery as needed, and perform other duties to advance IPHCCs Vision, Mission, and Values.

Skills and Qualifications
- Minimum bachelors degree in a Health-Related Discipline or an equivalent combination of training and/or relevant experience.
- Masters level degree or other postgraduate training in a related field is an asset.
- Ten or more years of experience in Senior Management.
- Experience managing various funding portfolios.
- Experience working with diverse communities that reflect IPHCCs priority populations.
- Knowledge of the social determinants of health and health equity.
- A solid background in community-based development, including experience in at least two of the following areas: primary healthcare, population health, youth and/or family services, support to racialized and/or vulnerable populations, community development, or community-based education programs.
- Experience in generating funding for programs, including writing proposals and/or generating fee-for-service programs that enhance the sustainability of the programs.
- Able to effectively communicate both verbally and in writing.
- Able to obtain a successful vulnerable sector screen.
