Reporting to the Director of Operations, the Intake Worker serves as the first point of contact for individuals seeking primary care in Niagara Falls, playing a key role in creating a welcoming and supportive environment. They will conduct initial needs assessments to ensure each individual's health concerns are clearly understood and appropriately addressed. Using a standardized assessment tool, the Intake Worker determines whether a person's healthcare needs are best met through the Community Health Centre (CHC) or are more appropriately served by a local family physician. They will also facilitate referrals and build strong relationships with primary care providers and community organizations to support integrated, client-centered care pathways.Main Responsibilities/Duties:
- Conduct intake assessments to determine the complexity of patient needs.
- Evaluate psychosocial, medical, and social determinants of health to support appropriate service referrals.
- Determine whether clients are better served within the CHC model or through referral to an external Family Health Team based on complexity tool
- Develop and maintain collaborative relationships with local family physicians and primary care teams to support seamless client transitions.
- Create and maintain efficient referral pathways and protocols between CHCs, FHTs, and other community health resources.
- Connect clients to appropriate internal/external programs and supports including mental health services, outreach supports, addiction services, and social services.
- Maintain accurate and confidential documentation of client interactions and referrals in electronic medical records.
- Participate in multidisciplinary team meetings and community outreach as needed.
- Monitor and evaluate referral outcomes and client satisfaction, adjusting pathways for improved effectiveness.
- Assist clients with completing necessary forms and documentation.
- Participate in team meetings and contribute to ongoing program development.
- Promote a culture of collaboration and respect across all disciplines and functions.
- Contribute to and support the implementation of the organization's strategic plan and priorities.
- Diploma or degree in Healthcare Administration, Social Services or a related field.
- Multilingual abilities are an asset.
- Strong interpersonal and communication skills, with the ability to work with diverse populations.
- Commitment to fostering an inclusive and culturally competent care environment.
- Maintaining an understanding of the health issues of the community to provide appropriate services.
- Proven ability to provide compassionate, respectful, non-judgmental and culturally competent care to the marginalized populations.
- Demonstrated communication, time management, problem solving and organizational skills.
- Previous experience establishing collaborative working relationships with a multi-disciplinary team, clients and community service providers.
- Previous experience in intake coordination or client services within a community health setting.
- Ability to problem-solve and apply sound judgment.
- Proficiency in the use of computer software/electronic medical records.
