Are you the one were looking for?

We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment to our clients, families, and each other.

TheIntensive Case Manager is responsible for managing the case load of up to 7 adult dual diagnosis clients in a permanent residential setting. In this position, you will be scheduled MondayFriday, 8am-4pm within a 24-hour program.

This position is aunionized, full-time, temporary position. The contract is until December 31, 2025. The rate of pay is $24.38 per hour and will go up to $24.38 starting April 2025. Hours per week are 40.

Key Responsibilities:

  1. Managing referrals as needed
  2. Accompanying clients to medical appointments and providing any necessary support
  3. Keeping client files up to date
  4. Helping clients work on self-identified goals
  5. Participation of on-call rotation
  6. Managing supply orders
  7. Assisting clients with medication management, meal preparation and maintaining their unit
  8. Other duties as assigned

What you will bring:

  1. Diploma in social services or equivalent
  2. CPR and NVCI certificate
  3. Minimum one-year experience working with dual diagnosis population
  4. Experience with case management in a residential setting
  5. Exceptional written and verbal communication skills
