As a Wellness Navigator specializing in senior care, you will be responsible for supporting and guiding older adults in achieving and maintaining their wellness goals. You will act as a resource, advocate, and coach, ensuring that seniors have access to the necessary tools and information to enhance their physical, mental, and social well-being. Your role will involve developing personalized wellness plans, conducting assessments, coordinating services, and providing ongoing support to empower seniors in leading healthy and fulfilling lives.
Responsibilities
  • Conduct comprehensive wellness assessments to identify seniors' strengths, challenges, and goals.
  • Develop individualized wellness plans in collaboration with seniors, taking into account their preferences and capabilities.
  • Provide guidance on nutrition, physical activity, chronic disease management, mental health, and social engagement.
  • Connect seniors with community resources, health services, and educational opportunities.
  • Advocate for seniors' needs and rights within the healthcare system and community settings.
  • Monitor progress and adjust wellness plans as needed to ensure continued improvement.
  • Educate seniors and their families on health-related topics and preventive care strategies.
  • Foster a supportive and trusting relationship with seniors to promote their overall well-being.
  • Maintain accurate records and documentation of interactions and outcomes.
Qualifications and Experience
  • Completion of Nursing diploma
  • Proven experience working with older adults in a healthcare, social services, or community-based setting.
  • Knowledge of aging-related issues, chronic diseases, and evidence-based wellness practices.
  • Strong interpersonal and communication skills, with the ability to build rapport and motivate others.
  • Empathy, patience, and sensitivity to the needs of seniors and their families.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in using computer applications for documentation and data management.
  • Flexible hours may be required to accommodate seniors' schedules, including evenings and weekends
  • Experience in case management or care coordination
