Position: Primary Care Nurse (Clinic Setting) St. Albert, AB

Job Description:
We are currently seeking a Primary Care Nurse with a strong background in outpatient or clinic-based care. The ideal candidate will be patient-focused, highly organized, and passionate about supporting individuals and families across the continuum of care. This role involves working closely with physicians, nurse practitioners, and allied health professionals to deliver comprehensive, team-based care.
Key Responsibilities:
Provide direct patient care including assessments, immunizations, injections, wound care, and patient education
Assist with chronic disease management (e.g., diabetes, hypertension, COPD) through care planning, health coaching, and follow-up
Triage patients and respond to phone calls, messages, and requests for information or advice
Prepare patients and exam rooms for physician or NP visits, and assist during minor procedures as needed
Maintain accurate and timely documentation in the EMR (Electronic Medical Record) system
Participate in quality improvement initiatives and clinic workflow optimization
Collaborate with internal and external providers to ensure continuity of care
Support health promotion activities and patient outreach programs
Qualifications:
Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the College of Registered Nurses of Alberta (CRNA) or College of Licensed Practical Nurses of Alberta (CLPNA)
Minimum of 2 years of clinical experience preferred, ideally in a primary care, family practice, or community health setting
Current CPR certification
Experience with chronic disease management and preventative care is an asset
Proficiency with EMRs (e.g., Telus PS Suite, Med Access, or similar)
Strong clinical judgment, communication, and interpersonal skills
Ability to multitask in a fast-paced, patient-centered environment
Commitment to maintaining confidentiality and ethical practice
