Position Summary

We are seeking a Mobile Diabetes Registered Nurse to join our team, working collaboratively to enhance the health, wellbeing, and self-management of clients living with diabetes.
Under the direction of the Supervisor, the Mobile Diabetes Registered Nurse provides culturally informed nursing care, chronic disease education, and health support services to adults and Elders. This role involves diabetes education, client assessments, health planning, and mobile outreach care. The uniqueness of this position lies in its mobile nature, which includes regular travel to First Nations communities across southern British Columbia to deliver in-community care and build long-term wellness relationships. The Mobile Diabetes Registered Nurse will also engage in community relations activities to support clients in the broader Seabird Island community and beyond.
What You'll Do
Clinical Care & Assessment
- Develop and implement individualized nursing care plans based on client needs.
- Maintain accurate and up-to-date medical records, including vital signs and health histories.
- Assess client health concerns and needs related to diabetes and general wellness.
- Perform point-of-care testing, glucose monitoring, A1C testing, and administer insulin and diabetes medications.
- Experience and proficiency with recommending adjustments to oral and injection medication used in the treatment of diabetes.
- Provide instant care during medical emergencies such as accidents, burns, strokes, and cardiac events.
Diagnostics & Referrals
- Make appropriate referrals to internal or external programs and health professionals.
- Coordinate with the supervisor and interdisciplinary team to assess, plan, implement, and evaluate client care plans.
Health Education & Promotion
- Teach patients and families about diabetes self-care, medication management, diet, and physical activity.
- Support patient independence by helping set health goals and answering condition-related questions.
- Provide culturally appropriate health education and wellness guidance to individuals and groups.
- Promote disease prevention and overall community health improvement strategies.
- Participate in and support health outreach initiatives and workshops.
Health Program Support
- Collect statistical data related to services, clients, and outreach outcomes for evaluation and reporting.
- Maintain nursing supply inventory and ensure timely ordering and restocking of materials and medications.
- Monitor, document, and report any changes in patient health conditions.
- Coordinate infection control protocols, including advising staff and clients on best practices.
Leadership & Mentorship
- Assist in supervising and supporting less-skilled healthcare staff and nursing assistants.
- Provide mentorship and education to students or new team members as needed.
- Collaborate with other members of the health team to ensure integrated service delivery.
Community Engagement & Support
- Establish a compassionate care environment that addresses emotional, psychological, and spiritual needs.
- Transport clients to and from health-related appointments and services as needed.
- Foster trust and strong relationships with clients, caregivers, and community members.
Professional Practice
- Maintain knowledge of current nursing practices by attending workshops and reviewing literature.
- Ensure professional, ethical nursing practices that align with BCCNM standards.
- Comply with all Seabird Island Band policies, procedures, and cultural protocols.
- Support continuous program improvement and respond to feedback.
Other Duties
- Additional responsibilities as needed to support the community and uphold our shared values.
Working / Special Conditions
- Initial and ongoing employment is subject to submission of a clear Criminal Record Review Program Check to ensure the safety and trust of the communities we serve. This check will be conducted every five years.
- Initial and ongoing commitment to the values and guidelines of Sq'wqel (Seabird Island Band), including our policies, procedures, standards of practice, and laws, is essential. This commitment supports our collective work, the community, and the responsibilities of this role.
- Must possess a valid Class 5 driver's license and be eligible to operate Seabird Island Band fleet vehicles, which are used to travel to and from communities throughout southern British Columbia to deliver mobile health services and support clients.
- Flexibility in working hours may be required, including evenings and weekends, to accommodate the needs and schedules of clients and community needs. Employees will be given as much notice as possible to any changes in schedule.
What You Bring
- A Diploma or Degree in Nursing, along with registration and licensing with BCCNM. We also recognize and value cultural knowledge and experience gained through other forms of education and training, including teachings from Elders and Knowledge Keepers.
- Formal education or training in diabetes education, chronic disease management, or a related health care field. Other forms of post-secondary education and training, including teachings from Elders and Knowledge Keepers, will be considered.
- At least 5 years of relevant experience working within Indigenous communities, focusing on diabetes care, chronic disease prevention, or mobile public health services.
- Experience working with Indigenous communities and understanding of Indigenous cultures, traditions, and contemporary issues.
- Excellent communication and relationship-building skills.
Preferred:
- Indigenous candidates are strongly encouraged to apply.

- Knowledge and understanding of Seabird Islands culture, traditions, teachings, community dynamics.
- Ability to respond quickly and professionally to community or colleague needs and inquiries.
- Excellent writing, editing, and verbal communication skills.

- Experience working within Indigenous communities and applying culturally safe practices.
- Understanding of intergenerational trauma and its impacts on Indigenous peoples and communities.
