TheHousing Support Worker (Leamington)plays a vital role in supporting individuals experiencing homelessness by facilitating access to shelter, housing, and essential community resources. This position requires strong assessment, advocacy, and case management skills to help clients navigate complex systems and work toward sustainable housing solutions.

What Youll Be Doing

  • Supporting individuals experiencing homelessness to secure and maintain housing.
  • Conduct pre-intake assessments using agency tools and a holistic understanding of each client's unique circumstances.
  • Liaise with community partners and external agencies to ensure coordinated and comprehensive client support.
  • Develop realistic and individualized housing search plans, considering both client needs and current market conditions.
  • Provide direct support to clients throughout the housing search and application process.
  • Build relationships and maintain ongoing communication with landlords, property managers, and housing providers.
  • Advocate on behalf of clients with landlords, service providers, and across various systems to address barriers to housing and support.
  • Monitor client progress, engagement, and potential risks using sound judgment and established assessment tools.
  • Make referrals to appropriate community programs and services to address clients broader needs.
  • Assist clients in accessing financial supports and income sources.
  • Collaborate with clients to develop and implement personalized housing and post-housing support plans.
  • Ensure adherence to program guidelines and regulatory requirements.

Outreach Responsibilities:

  • Maintain up-to-date knowledge of local (County) resources and services.
  • Monitoring areas through Essex County to identify and engage with individuals experiencing homelessness.
  • Build and sustain collaborative working relationships with community-based and County resources.
  • Complete rapid risk assessments to support effective service delivery and crisis response.

What You Bring

Post-secondary education in Social Work, Social Services, Human Services, or a related field (BSW preferred).

Minimum of 2 years of experience working with individuals experiencing homelessness, housing insecurity, or related social issues.

Strong understanding of housing systems, market trends, and barriers affecting vulnerable populations.

Knowledge and understanding of the By Names Prioritized List (BNPL) process, the SPDAT Assessment Tool, Central Housing Registry applications and HIFIS data system.

Knowledge of local community services, housing programs, and income support systems.

Demonstrated ability to perform assessments, develop support plans, and manage case documentation.

Excellent communication, advocacy, and interpersonal skills.

Strong crisis intervention and problem-solving abilities.

Ability to work both independently and collaboratively within a team.

Proficiency in Microsoft Office and client database systems.

Valid drivers license and access to a reliable vehicle may be required.

Position Details

  • Monday to Friday, with occasional evenings or weekends as needed. Schedule subject to change, based on the needs of the program, and occasional support with the overnight Warming/Cooling Centre may be required.
  • Pay Rate: $50,108 per annum ($27.53/hr)
  • Location: Leamington/Essex County
