We are hiring a Case Manager on our Early Psychosis Intervention Program!

Permanent, Full-time Position: 35 hours per week, evenings & weekends may be required.

Closing Date: July 16, 2025

Salary: $56,444-$58,504 per annum *The salary range offered represents the first 2 steps in a 7-step salary band. Candidate experience will be considered when determining the starting salary.

Location:

The successful candidates will work primarily from our Aurora Office (15150 Yonge Street, Unit 201, Aurora ON L4G 1M2) and out in the community for client visits and program needs, with regular travel in York Region & South Simcoe required.

Job Profile:

The Case Manager, works with clients 14 to 35 years of age who are experiencing, or in the early stages of, their first episode of psychosis. The Case Manager works collaboratively with the client and the team to assess, plan, develop, coordinate and provide treatment, rehabilitation and support. The purpose of early intervention is to utilize and support bio-psychosocial interventions, using natural supports to the greatest extent possible, to enable the client to maintain and develop age-appropriate functioning and roles.

The Case Manager collaborates with clients to provide individualized and flexible services to engage, support, and integrate them into the community, and facilitate their use of resources and supports to maximize their independence and potential. The Case Manager adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety. Other duties as required.

As well, the Case Manager participates in individual and systemic advocacy initiatives to ensure that clients' wishes are heard and addressed, and to improve resource access and availability. The Case Manager may be assigned to different programs as required based on need.

In addition to compliance with all organizational policies and procedures, the incumbent is also expected to behave ethically, demonstrate competence in effective communication and team building, demonstrate effectiveness in working in a collaborative environment, and demonstrate confidence in decision-making and in building relationships.

Requirements:
• Degree with four years' experience or Diploma with six years' experience in a community based mental health organization; experience in early intervention is preferred.
Current registration with OCSWSSW or a regulatory body under the Regulated Health Professions Act
• A second language (French, Cantonese, Farsi, Italian, Korean, Mandarin, Punjabi, Russian, Tamil, Urdu) reflecting the local community is an asset.
• Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPPA requirements.
• Knowledge of PSR and Recovery as guiding principles.
• Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with consumers.
• Extensive knowledge of supports and services in York Region and South Simcoe, including formal and informal resources.
• Knowledge of systemic issues such as poverty, unemployment, stigma, medications and the isolation felt by individuals with serious mental health issues and their families.
• Ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with consumers, maintain confidentiality, and engage in reasonable self-care strategies designed to reduce stress by balancing work/life responsibilities.
• Excellent oral and written communication skills
• Ability to demonstrate computer skills, specifically Microsoft Office and Outlook
• A valid Canadian Full G driver's license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients.
• A satisfactory and current Vulnerable Sector Screening (Police Check).
