Program Overview – Community Care
The Community Care Unit provides a holistic approach to care at home for seniors and caregivers who are vulnerable, at risk and/or marginalized. The unit provides a wide range of programs such as Assisted Living Services, Home & Cluster Care, Social Work, Meals-on-Wheels, Adult Day programs, and transportation, which enable seniors to be as independent as possible in their homes. The unit also provides a variety of socio-recreational/wellness programs which enhance the quality of life and promote the health of the older adult.
What You Will Do
  • Embrace a client-centred and culturally competent approach to provide exceptional personal care that respects the diverse backgrounds and needs of each client.
  • Efficiently set up and dismantle program activities, maintaining a clean and hygienic environment that prioritizes safety.
  • Assist clients with a range of Activities of Daily Living (ADLs), such as feeding, transfers, bathing, skin care, oral hygiene, toileting, and continence care, ensuring dignity and comfort.
  • Execute non-controlled acts as assigned by the Supervisor, strictly adhering to the established Unit's policies.
  • Perform controlled acts delegated by a Regulated Health Professional, following the Unit's policies.
  • Offer medication assistance and reminders in compliance with the Unit's Medication Assistance Policy, ensuring clients adhere to their medication schedules safely.
  • Engage in continuous observation of clients and their environments, promptly reporting and documenting any unsafe conditions or notable changes—be they behavioural, physical, mental, or cognitive—to the Supervisor.
  • Safely escort and transport clients to and from the program, ensuring their secure arrival and departure.
  • Diligently complete and maintain detailed progress notes in client profile binders and communication logs to ensure accurate record-keeping and information sharing.
  • Communicate openly and collaborate effectively with caregivers and stakeholders to determine the most effective care approaches for each client.
  • Respond professionally to any challenging behaviours presented by clients; if difficulties arise that cannot be managed effectively, report to the Supervisor for follow-up.
  • Apply proper food handling practices in alignment with the Unit's policy; prepare snacks and meals, assist clients who may require help with eating.
  • Monitor the supply of program materials, ensuring an adequate quantity of forms and equipment is available, and promptly report to the Supervisor when supplies are running low.
  • Actively contribute to achieving the anti-racism objectives and community development principles that guide the organization's mission within the program.
  • Assist Program Activationists in implementing engaging program activities as needed, promoting an enriching experience for all participants.
  • Display flexibility by being willing to transfer between Seniors' Day Program sites as operational needs dictate.
  • Additional training in dementia care is highly valued and considered a beneficial asset.

What You Bring to the Team
  • Personal Support Worker Certificate and NACC Certificate (if applicable).
  • Experience providing personal care to frail seniors and/or disabled adults.
  • Minimum one (1) year experience Providing personal care to seniors and people living with disabilities

What Will Set You Apart
  • Comprehensive understanding of how to support clients in meeting their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (iADLs).
  • Strong communication skills, encompassing verbal, written, listening, and reporting abilities, to ensure clear and effective interactions with clients and team members.
  • Ability to work collaboratively and effectively within a team, fostering a supportive and cooperative environment.
  • Flexibility and sound judgment in assessing and responding to clients' diverse needs.
  • Familiarity with behavioural management techniques and approaches to effectively assist clients in various situations.
  • Knowledge of gerontology issues, healthcare systems, and social services applicable to the demographic served.
  • Competence in food safety handling practices and maintaining a sanitary program environment.
  • Physical capability to perform frequent light to moderate lifting, walking, bending, stretching, and pulling.
  • First Aid and CPR certification is a valuable asset
  • Proficiency in Chinese (Mandarin/Cantonese) is considered an asset.
