Overview:
This member of the Canadian Mental Health Association, Kenora Branch works alongside a team of professionals whose focus is safety, stability and support for individuals at the Kenora Supportive Housing & Safe Bed Program operated through CMHA Kenora Branch. The Case Manager has the clinical skill and experience to assess, plan, develop, coordinate, connect and support services to program clients. The Case Manager will participate and be part of a team approach assisting individuals with developing their own recovery plan and coordinating services that support these goals. The position will be split between both the Kenora Supportive Housing Program and Safe Bed Program (typically working 8:30 am 4:30 pm, Tuesday & Thursday at KSH; Monday, Wednesday & Friday at Safe Bed); however, hours and days spent at each program are subject to change. This position reports directly to the Chief Executive Officer & Safe Bed Program Team Lead.
Qualifications:
BSW preferred, or demonstrated equivalent
Knowledge of mental health/illness and mental health difficulties and treatment modalities
Assessment, advocacy and evaluation skills
Ability to work collaboratively with clients, team colleagues, and community stakeholders
Ability to participate side by side with individuals to accomplish tasks, develop meaningful activities
Ability to carry out and model activities of daily living
Cultural Competence for diverse populations
Working knowledge of community resources and agencies
Ability to work independently and in a team environment
Computer literacy and recording skills
Valid Drivers Licence
Criminal Records Check
Certification in CPR and First Aid
Ability to work flexible hours
Ability to manage conflict and crisis situations
Networking skills and outreach skills
Ability to assess dynamic risk
Highly developed communication skills
Understanding and applied knowledge of Recovery Model & Strengths Perspective
Duties:
Adherence to all policies of the CMHA Kenora Branch
Assurance of confidentiality of all client, personnel and organizational records and information
Maintenance of client files and data files including updating of client information consistent with policies and procedures of the CMHA, Kenora Branch
Participation in training and educational opportunities and application of new skills to practice
Provide crisis intervention, advocacy and education as indicated
Assist in the development and monitoring of the recovery plan with each resident based on their own needs and preferences
Advocacy or linking of clients with appropriate services, supports and resources in order to meet the goals and objectives of the recovery plan
Function as a fully integrated member of the team for the client residing within the residence
Monitor house rules compliance and report any issues to the Chief Executive Officer
Interface with community services and programs with and on behalf of residents
Facilitate and monitor the implementation of client programs for the residents, including daily programming groups
Design and facilitate programming to enhance clients life skills (i.e. budgeting, healthy lifestyle, food preparation, household upkeep, personal safety)
Organize, facilitate and participate in monthly staff meetings and other treatment plan meetings as deemed appropriate in relation to the residents
Organize and facilitate information sharing sessions with clients regarding topics of interest they have identified in their recovery plans
Participate on specific Committees, tasks or work groups as directed
Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures; including but not limited to Fire, Disaster and Evacuation Plans
Ensure client safety and client safety activities are emphasized throughout orientation of new staff members
Support an environment which avoids, prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of mental health services
Participation in administrative and office functions
Modify working hours as needed to meet programming needs of the residents
Responsible for self-care
Other duties as directed
