Position Overview
We are seeking a passionate and dedicated Family Case Manager, Shelter to join our team at Inn from the Cold. The Family Case Manager, Shelter is dedicated to serving families from diverse backgrounds. The Shelter Family Case Manager, Shelter plays a critical role in helping families navigate their journey from shelter toward stability, self-sufficiency, and permanent housing. By integrating culturally appropriate service and support, this role strengthens family connections, fosters healing, and builds pathways to sustainable housing solutions.
What Youll Do
- Engage and build rapport with shelter families and meet with them regularly to provide support, develop goals and monitor progress toward stable permanent housing
- Conduct intakes and complete assessments to evaluate client needs, risks, and strengths to work with the family in identifying short-term and long-term goals
- Develop and implement personalized case plans with the family that prioritize mutually agreed upon stable and permanent housing solutions
- Provide advocacy & resource navigation support to address barriers and assist families in accessing culturally relevant community resources, housing programs, employment services, mental health support, health care and financial assistance
- Ensure continuous communication and collaboration with Inn from the Cold teams to ensure families receive comprehensive, wraparound support
- Collaborate with Inn colleagues to provide housing sustainability services including tax clinics, family budget support, employment workshops, and secure legal identification
- Complete applications for Income Support, Subsidized Housing and Fair Entry
- Develop and maintain a network of strong community partnerships and participate in community meetings as required
- Maintain accurate, timely, and confidential documentation of client interactions, case notes, case plans, safety concerns and records of support and service referrals using HOMES, Program Drive folders, and Microsoft Teams
This position description reflects the general duties of the role, however, it is not intended to include all duties that may be inherent in the position. The Shelter Family Case Manager, Shelter may be asked to perform other duties as required or assigned.
What You Bring to Our Mission
- Completion of a diploma or degree program related to social work, family case management, or human services is preferred
- Relevant and solid case management practice skills including issues related to homelessness and its impact on families
- Knowledge and experience of Indigenous cultures and worldview, community and history
- Knowledge of trauma-informed and client-centered care with ability to respond to the complex needs of families in a family shelter setting
- Knowledge of housing-focused community resources, services and support
- Current First Aid/CPR is required; Suicide Intervention (ASIST), Self-Harm and Crisis De-escalation training is an asset
- Proficient documentation including case notes and case plans, and survey reporting skills using a client information management system (HOMES); solid understanding on confidentiality
- Computer literacy skills are required, including the ability to use MS Office 365 and Teams
- Excellent communication, problem-solving, and teamwork skills
- Able to communicate effectively both verbally and in writing in the English language
