Position: Case Coordinator (Intake and Engagement)

Position Type: Full Time

Salary: Case Coordinator Salary Grids ($52,032-$60,150)

Position Description:

The Case Coordinator (Intake and Engagement) performs a range of activities including: completing intakes with clients; leading Engagement Groups; new client entry into EMHware; maintaining accurate attendance at Groups in EMHware; supporting colleagues to manage the wait list for our Clinical Services; pre-treatment care planning; coordinating admissions to bed-based programs to ensure full program occupancy; collection of necessary client information to support the intake process; collaboration with community and health partners; assessments; brief interventions; individual counseling; group facilitation; case management functions for Engagement Program participants; working with a student placements.

The key responsibilities of the Case Coordinator include:

  • completing intakes, enrolling clients in EMHware and on the appropriate waitlist;
  • managing bed intakes to avoid vacancy;
  • maintaining accurate records of all Engagement client contacts;
  • being the Case Coordinator with primary responsibility for assisting clients on the waitlist in preparation for admission to our bed-based treatment programs;
  • working with clients in developing and modifying individual Recovery Care Plans prior to admission;
  • conducting formal assessments in the areas of substance use and other health issues, including Intake Assessments and the GAIN-SS;
  • facilitating Engagement Groups;
  • working with partner organizations and client supports (including drug court staff, family, primary care providers, referring agency staff, social workers in the prison system, defence lawyer) to support the coordination of client admissions to bed-based programs, including pre-treatment stays in WMS or other addiction service locations;
  • working with partner organizations and referring agency staff to support the coordination of client access to waitlists for the Psychotherapy, Transitional Housing, Continuing Care, and Contingency Management Programs;
  • utilizing EMHWare to its full capacity for admission, case notes, discharge and management of the client file.

Additionally, the Case Coordinator responsibilities include, but are not limited to:

  • informal assessment;
  • brief interventions;
  • individual counseling;
  • assisting clients with challenges related to the social determinants of health;
  • information/health promotion;
  • referral;
  • assisting clients with their medications; and,
  • liaising with community and allied services, resources, partners and referral sources.

Knowledge, Skills and Abilities

  • Knowledge of issues related to addiction, concurrent disorder, mental health, trauma and recovery.
  • Knowledge and understanding of community housing, addiction and mental health services.
  • Knowledge of the spectrum of harm reduction and abstinence based philosophies.
  • Strong skills with EMHware or other electronic records database.
  • Experience working in a residential setting with men living with addictions and/or mental health.
  • Experience utilizing various approaches to housing and substance dependence.
  • Extensive knowledge and skills in building rapport, assessment, supportive counseling, case management and brief-intervention.
  • Knowledge of the social determinants of health.
  • Knowledge, understanding and sensitivity to the complexity of issues arising for individuals who have experienced homelessness, addictions, poverty and/or mental illness.
  • Ability to work collaboratively within and across interdisciplinary teams and community partners.
  • Ability to work with clients presenting with a wide range of addiction and mental health issues.
  • Demonstrated flexibility, sound judgment, initiative and creativity.
  • Experience working within an anti-oppression and trauma informed framework.
  • Excellent interpersonal and communication skills.

Qualifications

  • University degree in a related field OR equivalent education and related work experience.
  • Membership in good standing, with a regulated health profession including the Ontario College of Social Workers and Social Services Workers, the College of Registered Psychotherapists of Ontario, or the College of Nurses of Ontario preferred.
  • Demonstrated experience working with individuals with dealing with addictions or concurrent disorders.
  • Demonstrated experience in screening and assessment, supportive counseling, case coordination/management and brief intervention.
  • Demonstrated experience in motivational interviewing.
  • Demonstrated experience with group development and facilitation.
  • Ability to work in a language in addition to English is a definite asset.
