About the Role:

The Permanent Supportive Housing (PSH) Program provides a long-term, affordable housing solution with flexible and voluntary support services offered to support high-acuity individuals who are homeless with housing retention and independent living.

The Group Facilitators primary responsibilities are to develop, implement, oversee, and facilitate the various psychoeducational groups in the program. This role will also support with ensuring the PSH Program Model is upheld at all times, by providing feedback to staff and residents.

Role Type: Full time, Permanent

Hours of Work:

  • Days (6am-4pm)
  • Hours are scheduled on a four (4) week rotational basis which includes working every second weekend, and occasional holidays (Statutory holidays)
  • Participation in an on call rotation is a requirement of this role, in addition to the regular hours of work

Qualifications:

  • Completion of a College Diploma or University Degree relevant to mental health, addiction, or Social Services
  • Minimum one year of progressive experience in addictions or mental health; experience in concurrent disorders and trauma informed practice an asset
  • Previous experience in housing support an asset
  • Previous experience with justice-involved individuals preferred
  • Food Handlers Certification an asset
  • Lived experience related to addictions and/or mental health an asset
  • CSC Clearance conducted by PWGS Canada
  • Standard First Aid/CPR, Level C
  • Valid G Drivers License (G2 or G) and Satisfactory Motor Vehicle Record
  • Reliable vehicle and willing to transport program participants
  • Must provide proof from Insurance provider of 1) at least $2 million in general liability and 2) acknowledgement of scope of vehicle use by Insurance provide
  • Mandatory on call duty, on a rotational basis
  • Must be able to attend work (travel to PSH building) within an hour of notification, when on call

Role Overview:

Psychoeducational Programming:

  • Develop, coordinate and facilitate daytime and evening psycho-education groups, including but not limited to:
    • Life skills
    • Relapse Prevention
    • Creative Coping
    • Emotions Management
  • Maintain and provide oversight of psycho-educational content
  • Develop a roster of appropriate community resources and guest speakers for educational seminars and workshops
  • Establish and maintain inventory of program supplies
  • Provide individual follow up after groups, when indicated

Residential Responsibilities:

  • Provide mental health and crisis support
  • Participate in supervision
  • Provide peer mentorship and coaching to the team
  • Conduct/inform staff briefing in between shifts
  • Transport tenants to appointments and other needs
  • Provide light housekeeping
  • Participate in program and team meetings
  • Conduct unplanned tenant discharges
  • Identify and mitigate tenant risks
  • Collaborate with external partners
  • Attend all mandatory training programs
  • Rotate attendance at the weekly staff meeting
  • Facilitate Crisis Intervention and supportive counseling/containment
  • On a rotational basis, provide on-call services during evenings, overnights and weekends

Administrative Responsibilities:

  • Complete tenant documentation and other required documentation (example: Electronic Log, program summary documents, Incident Reports, etc.)
  • Make referrals to internal and/or external programs or services, as appropriate
