Job Description

The Program Director for Ambulatory Care will provide strategic and operational leadership for the Ambulatory Care program across all Halton Healthcare and community sites. This role requires a collaborative leader who will work closely with the Program Medical Director to drive the program's strategic vision, ensure high-quality patient care, and promote effective resource management. The Program Director will also shape organizational culture, foster cross-functional collaboration, and support corporate projects impacting organizational and patient care outcomes.

Specifically, the Program Director will:

  • Develop and implement a strategic plan for the Ambulatory Care program aligned with the organization's overall strategic plan and vision.
  • Assess and address the need for new or expanded services based on performance analytics and evolving patient needs.
  • Work collaboratively with the Program Medical Director and other key stakeholders.
  • Coordinate internal and external planning efforts to support regional and provincial initiatives.
  • Facilitate the integration and redesign of services to enhance program effectiveness.
  • Establish Program goals that are aligned with the organization's strategic priorities
  • Shape organizational culture and climate by creating an environment that aligns with HH's values, promotes equity and inclusion, supports the interdisciplinary teams and enables continuous quality improvement.
  • Facilitate cross-functional collaboration with clinical teams to ensure timely and appropriate patient care; ensuring patients and families are supported and informed throughout their care journey.
  • Support fundraising efforts and represent the organization at regional and provincial levels.
  • Use data analytics to understand utilization trends and performance management.
  • Ensure the program focuses on quality patient care and adheres to best practices.
  • Monitor performance indicators and develop action plans for continuous improvement.
  • Address patient safety concerns and implement risk mitigation strategies; support quality review processes and investigate critical incidents.
  • Coordinate and align resources through effective planning and budgeting, to ensure the delivery of high quality, evidence based patient care in the most cost effective manner possible.
  • Support corporate projects that impact organizational functioning and/or patient care.
  • Participate in corporate committees and initiatives, and seek feedback to refine leadership approaches.
  • Foster an environment of learning and development for Program staff through coaching, managing performance and mentoring.
  • Cultivate a positive labour relations environment within the Program and applies policies and appropriate collective agreement language consistently and fairly.
Qualifications
  • Master's degree in Business, Healthcare Administration, Nursing, or a related field.
  • Current registration with the College of Nurses of Ontario or other relevant health regulatory college.
  • Minimum of 7 years of clinical and administrative leadership experience in an acute care medical setting.
  • Recent experience managing outpatient or ambulatory care programs.
  • Proven ability to build collaborative relationships and optimize communication strategies.
  • Demonstrated success in setting goals, resolving conflicts, and managing change.
  • Expertise in resource allocation, financial management, and quality improvement.
  • Ability to foster a supportive learning environment and drive organizational performance.

Salary Range: $141,738 - $177,173
