The London HART Hub is a low barrier recovery-oriented space that aims to support individuals experiencing homelessness with mental health and addiction needs to improve their health and wellbeing. HART Hubs will support its clients through access to high quality evidence informed care that is respectful, trauma-informed, culturally appropriate and compassionate in nature. It connects individuals with a continuum of care tailored to their needs by providing access to transitional supportive housing spaces where individuals can access safety, basic needs, health care and other mental health and addiction services under one location.
The System Navigator will provide client-centered, community-based services to participants of the London HART Hub, assisting them in navigating the health and social system supports available to them. The position will include individualized assessment in planning, monitoring and support, systems advocacy and resource coordination.

WHAT WE'RE OFFERING:
- What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement
- When you'll work - 70 hours over a two-week period; includes Days/Afternoons/Evenings including weekends

- Your pension plan – Immediately eligible for optional enrollment in the Healthcare of Ontario Pension Plan (HOOPP)
HOW YOU WILL BE MAKING A DIFFERENCE:
- Assist and advocate for/with clients to navigate systems, connect to appropriate resources, obtain needed services and supports, and to complete referrals to external services or to adhere to court or probation conditions (where applicable)
- Provide and facilitate referrals to mental health and addiction resources as appropriate.
- Assess situations requiring specialized services and arrange appropriate linkage and support
- Use standardized tools (GAINSS, OCAN, LOCUS, VI-SPDAT) and their results to determine program eligibility and help the client identify their goals and preferences
- Advocate on behalf of clients (e.g. with hospital and community services, landlords, boarding houses, police) to ensure the client's rights are not compromised
- Offer treatment options, wellness planning and other strategies based on client's intentions and stated needs
- Willingness to attend to clients who are encamped or sleeping rough to offer support, referral and intake at emergency shelters
- Assist clients, when required, to deal with environmental difficulties and to develop social living skills conducive to community living
- Encourage and facilitate personal growth and development towards recovery by providing individual skill teaching and counselling; motivate the client to obtain the necessary skills and behaviours through training from other sources
- Provide health and wellness support including participation in and/or co-ordination of Community Care Plans (CCPs), relapse prevention and planning, wellness/recovery planning, medication education, symptom and medication monitoring, referrals to specialized health services, and support with physical health concerns
- Provide needed information and education to key stakeholders about mental illness, addiction, coping skills, services, resources, self-help and consumer initiatives/supports
- Meet with client regularly to work on specific goals and adjust interventions until other permanent supports are in place
- Ensure client record is updated as per organization standards, discipline-specific guidelines and standards and PHIPA legislation, and handled in accordance with applicable privacy laws and organization policy.
- Provide support for additional HUB sites as required.
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
- Post-secondary degree or diploma in a program relevant to mental health or social services. (Primary proof of credentials will be required during the hiring process, i.e. an original degree/diploma/certificate or official transcript; for ongoing education, an unofficial transcript will be accepted)
- Valid Drivers Licence and Access to a vehicle with adequate insurance to carry out job duties and occasionally transport clients
We prefer the following qualifications:
- Experience working with individuals with addictions or problematic substance use; mild, moderate and serious mental illness; no fixed address
- Minimum one-year full-time employment experience in delivery of mental health, addition, or community-based intervention services
- Extensive knowledge of community resources, the legal system, service & support programs, and the various barriers clients face to service and maintaining stable housing and personal wellness.
- Residential/community housing experience and familiarity with the Residential Tenancies Act.
- Experience in crisis intervention/management and crisis resolution.
- Valid driver's licence and access to vehicle.
- First Aid & CPR, ASIST, and CPI certifications
