Our Resident Support Coordinators support and foster a holistic and resident-centered care approach that includes advocacy, education and communication on behalf of residents and their families. To promote an optimal quality of life for residents, by building on a person's strengths with dignity and respect and ensuring their rights are honoured.

Our New Village of Ridgeview Court, located in Milton, Ontario has an exciting opportunity for a RESIDENT SUPPORT COORDINATOR (externally known as a Social Worker or a Social Service Worker) to join their village team.

Start date for this role is Summer 2025

As our new Resident Support Coordinator, you will:

  • Coordinates and supports the resident move in experience, care conferences
    and applicable family meetings.
  • Support resident and their families with move-ins and transitions within the
    Village and/or throughout the health care system.
  • Support Village waitlist in partnership with the clinical team.
  • Provide ongoing assessment of needs, counselling, mediation and advocacy for
    resident well-being in collaboration with the Village inter-disciplinary team.
  • Provides resident guidance in significant areas such as loss, life transitions and
    areas around the Resident Bill of Rights.
  • Engage with outside agencies and community groups as resources to offer
    support to residents and/or families comprehensive plans of care.
  • Serves as a key representative for Residents' Council and Family Council.
  • Provides education, coaching, and counselling for residents, families, and team
    members regarding mental health and well-being.
  • Assist in the development of effective communication strategies for residents
    and their families (will support mediation if appropriate).
  • Perform other duties, appropriate to the position, as assigned.

Our new Resident Support Coordinator must have:

  • A Post-secondary education in Social Work or Social Services Worker Program
  • Must be registered with the Ontario College of Social Workers and Social
    Service Workers
  • Current Heart Save Level of CPR is an asset

It would be an asset if our new Resident Support Coordinator has:

  • Knowledge of relevant policies, procedures, legislation and regulations applicable to the long-term care sector, including the Fixing Long-Term Care Act, 2021 and Health & Safety Act of Ontario
  • Demonstrated experience working with seniors and/or persons with cognitive or physical impairments
