Albion Neighbourhood Services (ANS) provides critical support to individuals experiencing homelessness within Wards 1, 2, 3, 4, 5, 7, 8, and 9. Our Street Outreach Program connects clients with housing supports, emergency shelters, health services, and other community resources to foster long-term housing stability and self-sufficiency.

Position Summary:
We are seeking an experienced and motivated Street Outreach Team Lead Evening Shift to oversee and coordinate the evening outreach operations. This role ensures effective service delivery for individuals experiencing homelessness, leads the evening and relief outreach teams, and acts as the primary liaison with the City of Torontos Streets to Homes (S2H) program.
The Team Lead will be responsible for their team coordination, to attend to the client needs and Concerned Citizen Calls (within the mandated 4-hour window), managing case records, and maintaining program and health and safety compliance. They will lead ANS van-based outreach teams, engage directly with clients, and facilitate connections to housing, shelters, and community supports.
This role reports directly to the Housing Manager

Job Conditions:
Available for Evening Shift (MondayFriday, 3:00 PM10:30 PM)
Maintaining a clean driving record is mandatory requirement and will be periodically verified through the agencys insurance provider.
A Vulnerable Sector Police Check is required and subject to annual re-screening.
Outreach work requires navigating various environments, including outdoor encampments, shelters, and public spaces across all seasons, during evening and night hours.

Key Responsibilities:
Program Leadership & Coordination:
Lead and coordinate the evening Street Outreach Teams, including relief workers, ensuring high-quality service delivery and continuity.
Act as the primary liaison with the City of Torontos Streets to Homes (S2H) program, attending meetings, ensuring compliance, and disseminating updates to staff and management.
Coordinate responses to S2H inquiries and Concerned Citizen Calls within the required 4-hour timeframe.
Manage staff scheduling and shift coverage, ensuring alignment with program guidelines and funding requirements.
Oversee the maintenance and safe operation of the agency van, ensuring readiness for outreach, client transport, and fieldwork.
Case Management & Client Support:
Lead van-based and foot outreach, engaging with individuals experiencing homelessness in encampments, shelters, and public spaces.
Assist clients in securing and maintaining housing, including housing searches, landlord mediation, ID replacement, and barrier resolution.
Support clients in completing Rent Caf (RGI) applications, supportive housing applications, and resolving housing-related issues.
Provide housing stabilization supports, including training staff on accessing HSF, COHB, Bridging Grants, and other financial benefits for clients.
Provide harm reduction supports, including stocking and distributing naloxone kits, PPE, and other supplies.
Provide crisis management support to the client and teams if required.
Compliance:
Support evening and relief staff, ensuring adherence to agency policies and procedures as well as funders requirements.
Support teams with SMIS, TGRIP, and other case management systems to ensure timely, accurate client documentation (intakes, case notes, housing modules, discharges).
Ensure staff participate in meetings, training, and share updated calendars with leadership.
Maintain health and safety standards, identifying and reporting risks, and supporting compliance efforts with the Manager.
Ensure data tracking, timely reporting, and statistics submission for funder and agency requirements.
Community Engagement & Reporting:
Maintain effective liaison with City of Toronto S2H supervisors, local businesses, referral agencies, and partner organizations.
Provide program updates and respond to information requests from the City of Toronto and other stakeholders.
Coordinate enhanced services during extreme weather alerts (cold/heat) by adjusting shift coverage and outreach operations.
Support program evaluation and participate in meeting as requested.
Other duties as assigned.

Qualifications and Requirements:
Post-secondary degree or diploma in Social Work or a related field, or equivalent combination of education and experience.
Minimum 5 + years experience working with street outreach or homelessness-focused programs.
Minimum 3 years team lead, supervisory or staff coordination experience.
Minimum 3 years experience providing intensive case management to clients with complex needs (mental health, substance use, homelessness).
Strong oral and written communication, interpersonal, and conflict-resolution skills.
Demonstrated problem-solving and decision-making abilities.
In-depth knowledge of homelessness, housing systems, harm reduction, mental health, and substance use.
Familiarity with community resources, including housing, health, income supports, and harm reduction services.
Valid G-class drivers license and clean drivers abstract (required).
Ability to drive outreach vans in all seasons and weather conditions.
Vulnerable Sector Police Check (required prior to employment and subject to periodic reviews).
Highly reliable and able to work under minimal supervision.

Work Schedule:
Monday to Friday, 3:00 PM 10:30 PM
Work required for statutory holidays and agency closure days as needed.

Compensation:
$36.80 per hour ( includes pay equity)
