OVERVIEW: Reporting to the Sacred Care Program Manager, the Sacred Care Case Manager will be responsible for overseeing and supporting services provided at Sacred Care Homes. The Sacred Care Case Manager will work within a team that provides comprehensive case management services and supports a holistic circle of care. Shawenim Abinoojii is dedicated to the sacred duty of loving, protecting, and nurturing First Nation children and their families. The Sacred Care Case Manager will support youth according to the seven sacred teachings. Shawenim Abinoojii supports nurturing homes that honor the cultural identity, community connection and individual needs of youth.

KEY RESPONSIBILITES:

  • Provide on-call for assistance after hours and follow up with concerns or incidents that occur in the homes.
  • Crisis Intervention Providing immediate support and assistance during crises or emergencies, including connecting caregivers with emergency services, evacuations, or wellness support. providing support, and mobilizing assistance as needed.
  • Working closely with assigned Case Managers to conduct check-ins, ensure homes adhere to safety and case plans, and assist with paperwork.
  • Respond to calls collaborating with the Support Work Program to ensure that respite and support worker roles are adequately filled and scheduled and managing conflicts within the home.
  • Monitor and follow up with ongoing needs in the homes.
  • Share the incident reports with case managers for follow up.
  • Complete detailed case notes for on-call.
  • Communicate to Program Manager the observed training needs for House Parents, Support and Respite Workers.
  • Ensure all Foster Home documentation requirements, including reporting, is completed in a timely and accurate manner.
  • Other duties as assigned.
  • Regular travel is required for this position (Winnipeg and surrounding areas).

QUALIFICATIONS:

  • BSW degree or equivalent (similar education and experience working in the field will be considered).
  • 2-3 years experience.
  • Knowledge in the areas of colonization and intergenerational trauma.
  • Excellent conflict resolution skills.
  • Knowledge of the Child and Family Services (CFS) sector and legislation.
  • Demonstrate knowledge and/or understanding of First Nation perspectives, culture and traditional philosophies.
  • Experience developing and supporting a strong, respectful team-based approach to service delivery.
  • Experience working with families and navigating complex situations.
  • Satisfactory Child Abuse, Criminal Record check (with Vulnerable Sector), and Prior Contact Check.
  • Strong proficiency in Microsoft Office, word processing (MS Word), Spreadsheets (Excel), E-mail (Outlook) and Internet.
  • Excellent communication, organization, problem solving and team skills.
  • Ability to speak Anishinaabe is an asset.
