Position: Community Health Care Coordinator

Status: Full-time Term Position, 35 hours per week,

Schedule: Core hours: 8:00am-3:30pm

Reporting to: Director of Health

Salary: $41,769 /per year

Summary of Position:

The Neskonlith Indian Band is seeking a Community Health Care Coordinator. The Community Health Coordinator is a key member of the Wellness team, and reporting to the Director of Health, this position is responsible for coordinating individual and family needs through assessment and referrals to appropriate staff or agencies. The Community Health Coordinator is responsible for assessing clients for referrals to proper medical practitioners and other health care providers; this will include client intake and referrals to on-to-one counselling to determine clients' health care needs. The Community Health Coordinator has a responsibility in providing awareness, promotion, prevention, intervention, education and advocating for all community members in relation to improving their health care needs.

Qualifications & Requirements:
  • ability to work with families, including parents, grandparents, and children.
  • ability to assess individual/family strengths and provide health programming and support.
  • ability to provide referrals and follow-up for services.
  • ability to provide one-on-one coaching and group facilitation.
  • experience providing support, guidance, and education to families.
  • experience maintaining confidential client records and maintaining a resource database.
  • demonstrate awareness of the Neskonlith Community, culture and Secwepemc values.
  • Ability to establishing a relationship with individual families, assessing their needs,

Education and Experience:

  • Two (2) year minimum experience as Coordinator, within the Health field is an asset.
  • Must possess and maintain a valid Class 5 BC Driver's License and reliable vehicle.
  • Successfully pass Criminal Record Check for individuals working with youth or vulnerable adults
  • Valid First Aid, or willingness to obtain,
  • Excellent organization skills,
  • Excellent knowledge in areas of health and wellness,
  • Knowledge of Band office procedures and experience working in a First Nations Community
  • Please note: an equivalent combination of education, training and experience could be considered.
