Position Overview

Collaborates with the resident, family/caregiver, and health care team in the development, implementation and evaluation of the residents' plan of care. Provides leadership and mentorship to staff that enhances their knowledge, assessment skills, and ability to care for residents. Leads and collaborates in research, education, and evidence-based practice initiatives to optimize the resident, LTCH and health system outcomes.

Essential Duties

  • In collaboration with the Medical Director and Attending Physicians, provides comprehensive primary care to residents
  • Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding residents' plan of care,
  • Diagnose residents, order and interpret diagnostic tests, and prescribe medication and other treatment,
  • Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up
  • Provide direction and leadership to RN's, RPN's, PSW's, and other direct care staff in all aspects of resident care, within established legislative standards,
  • Makes referrals to specialized consultants, services, and other health providers,
  • Complete nursing documentation, and assist residents with move-in,
  • Is inspirational and passionate about quality dementia care and capturing positive moments
  • Work collaboratively with attending physicians,
  • Contribute positively to a team approach in being person centred with one another,
  • Ensure medication reconciliation occurs as needed,
  • Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research
  • Implement resident-focused programs, policies, and procedures with the Director of Care and the Quality Team,
  • Provide education to clinical team and offer in-services,
  • Liaise with Residents, families and outside agencies as required Respects the Residents' Bill of Rights
  • Any and all other tasks as assigned

Qualifications

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Post-graduate education and experience in gerontology (recommended)
  • Specialty certification in gerontological nursing (recommended)
  • Comprehensive knowledge of evidence based clinical practices
  • Comprehensive knowledge of long-term care concepts and philosophies
  • Experience in long-term care, with providing empathetic care for the elderly,
  • Excellent interpersonal and communication skills
  • Experience providing leadership and supervision in any healthcare environment.
  • Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
  • Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory reference
  • COVID-19 vaccination is a condition of employment
