We are growing and with 100+ team members. We are seeking an exceptional Nurse Practitioner (NP) to provide high quality, compassionate care to our patients and work collaboratively with our Cardiologists and other healthcare professionals in the management of complex cardiovascular cases to ensure the best patient experience out of any other medical clinic in Canada.

Responsibilities:

Working closely with the assigned Cardiologist, the principle responsibilities of the NP may include but are not limited to the following:

  • Conduct comprehensive patient assessments, take medical histories, perform physical examinations, and interpret findings
  • Order diagnostic tests and review incoming results
  • Maintain accurate and up-to-date patient records using electronic medical records (EMR) systems
  • Provide education and preventative care to patient
  • Formulate clinical treatment plans
  • Promptly follow up on incoming results
  • Communicate effectively with patients, families, and interdisciplinary team members
  • Administrative duties, including form completion, prescriptions renewals, and other administrative duties as deemed necessary for efficient clinic operation

Education and Qualifications:

  • Current and valid Nurse Practitioner license in Ontario
  • Certification from an accredited Nurse Practitioner program
  • Minimum three (3) or more years of experience as a Nurse Practitioner in a cardiovascular setting
  • Demonstrative expertise in cardiovascular assessment, diagnosis and treatment
  • Strong clinical, organizational, interpersonal and communication skills
  • Ability to provide patient focused provision of service
  • Ability to multi task within a fast paced environment
  • Excellent time management skills, ability to work independently, professional and courteous
  • Ability to use good judgement with excellent decision-making and problem-solving skills
  • Ability to work collaboratively with all One Heart Care team members
  • Ability to use the information technology provided
  • Proficiency in Microsoft Office, EPIC and electronic medical records (EMR) systems . Working knowledge of EPIC and/or Cerebrum an asset.
  • Adherence to PHIPA (Personal Health Information Protection Act)
