Permanent - Full Time

$20.79/hour

The Role: The Social Wellbeing Support role contributes to the mission of Community Support Connections by being responsible for providing support for different programs under the Social Wellbeing service umbrella. They work collaboratively with the Community Connectors and community partners to plan and coordinate social well-being programs, facilitating support and education groups for caregivers, attending community events and speaking engagements, supporting agency engagement efforts and outreach to leverage and strengthen connections with community partners and provide service coordination to clients.

The successful applicant will:

  • Work collaboratively with:
    • All Community Support Connections teams to build and sustain our ability to meet current, emerging and future client needs;
    • Community partners to maximize service delivery opportunities, identify and address gaps in service;
    • Clients and caregivers to provide social wellbeing programs
  • Engage and respond to current and potential clients by:
    • Facilitating telephone, virtual and in person programming including Friendly Visiting, Caregiver Support and Reassurance Calls;
    • Back up the Community connector role as needed
    • Community education and presentations
  • Accurately record client information in a timely manner.

Education/Qualifications

  • Diploma/degree/certificate in a related field and 2-3 months related professional experience or equivalent combination of education and experience
  • Ability to speak languages other than English (especially Portuguese, Spanish, Serbian and/or Romanian) considered an asset.
  • Experience working with older adults and/or people with diverse abilities
  • Developed assessment skills
  • Valid Driver's License and daily access to a reliable vehicle for business-related travel
  • Current First Aid/CPR A Certificate

Working Hours & Environment

  • Willing and able to work occasional evenings and weekends(community engagement)
  • Extensive computer and telephone use
  • Willing and able to work occasionally from a home office
