Join our dedicated team as a Family Practice Nurse (FPN) and play a pivotal role in delivering comprehensive, patient-centered care within our dynamic primary care team. As an FPN, you will be instrumental in enhancing our patients' health and well-being through a collaborative approach that emphasizes early detection, chronic disease management, and continuous care coordination. Your expertise will contribute to expanding our team's capacity by managing the care of 400 additional patients.

This position offers the opportunity to impact community health across multiple sites, requiring travel within the region to reach remote areas and serve priority populations. As a Registered Nurse in Family Practice, you will uphold the CRNNS Standards for Nursing Practice and the Code of Ethics, ensuring that our core values of leadership, organizational health, ethics, safety, and quality are reflected in your work and communication.

DUTIES AND REPSONSIBILITES

In this role, you will:

  • Provide ethical, compassionate, and competent care in a small community setting.
  • Engage in health promotion, screening, and disease prevention activities.
  • Foster long-term therapeutic relationships with patients and their families.
  • Collaborate with an interprofessional team to ensure the delivery of person and family-centered care.
  • Utilize your assessment and advocacy skills to contribute to community health.
  • Offer patient education, liaise with community resources, and coordinate clinic activities.

REQUIREMENTS, QUALIFICATIONS, & CONDITIONS OF EMPLOYMENT

  • Completion of the Family Practice Nursing Program or an equivalent recognized program is mandatory.
  • A Bachelor's degree in Nursing from an accredited institution is required.
  • Current registration as an RN with the College of Registered Nurses of Nova Scotia (CRNNS), or eligibility for registration.
  • Experience in a Mi'kmaq community is required.
  • Membership with the Canadian Family Practice Nurses Association (CFPNA) and Family Practice Nurses Association of Nova Scotia (FPNANS) is considered an asset.
  • Prior experience in a primary care collaborative team setting or a relevant practice area is preferred.
  • Proven ability to conduct comprehensive client assessments, care planning, and case management.
  • Effective communication skills with patients, families, the public, and healthcare professionals.
  • Strong critical thinking skills and clinical judgment.
  • Leadership qualities, ethical integrity, and a dedication to professional development.
  • A deep understanding of health promotion, population health, and health determinants.
  • Skills in health education and patient counseling, utilizing innovative technologies and strategies.
  • Experience in facilitating educational programs and group meetings.
  • Competence in time and resource management, fostering teamwork and interdisciplinary collaboration.
  • A commitment to fostering a positive work environment and supporting the professional growth of peers and students.
