HOURS - Regular Full Time - 8 hr shifts at 7.5 hours per day with a 30 minute unpaid lunch. Some flexibility for weekends shifts is required.

How You Will Make an Impact

You are a licensed Registered Practical Nurse (RPN), will work to their full scope of practice as an integral member of the intradisciplinary team in our Seniors Program. You will provide support to older adults (55years+) living with chronic health issues and social challenges in the Hamilton downtown area.

This role facilitates the daily functioning of this team to support clients to achieve and maintain an optimal level of health, well-being and age in place while accessing health care resources in an efficient and effective manner.

You will provide individualized support services to clients in the home setting and support them in problem-solving, safety planning and accessing essential resources; additionally, you will work collaboratively with their multi-disciplinary team members and provide primary support to a team of Personal Support Workers in providing quality care 24 hours/day, 7 days/week.

As well you will support clients, advocate and collaborate with HNHB Home and Community Support Services, primary care providers and other community partners in order to facilitate timely, high-quality care in the community and ensure effective and seamless transitions.


What You'll Bring to Our Team

  • Current Registered Practical Nursing registration in good standing with the College of Nurses of Ontario
  • Demonstrated confidence working autonomously and collaboratively with members of an interdisciplinary team
  • Excellent critical thinking skills and ability to use judgement to assess and intervene in critical situations
  • Demonstrated ability to practice within the professional boundaries and the therapeutic nurse client relationship in accordance with the standards of the College of Nurses of Ontario.
  • Experience working with persons with chronic health issues, fragility and other common issue related to aging.
  • Experience in wound care, an asset.
  • Excellent communication and interpersonal skills
  • Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd Centres; including the application and understanding of culturally sensitive approaches in relationships with co-workers and clients
  • Works well with volunteers and has demonstrated skills to lead a team, diffuse conflict and manage an environment
  • Ability to comply with all federal, provincial and municipal regulations and legislation
  • Computer literacy including: electronic medical records, Outlook, Microsoft applications
  • Fluency in a language in addition to English is an asset.
