West Bedford Transitional Health is Nova Scotia's first Nurse Practitioner led transitional care inpatient facility, delivered in partnership by Shannex and Nova Scotia Health. Our transdisciplinary team provides specialized, person-centered care that helps patients safely transition from hospital to home and community living. About the Opportunity
Nurse Practitioner's at West Bedford Transitional Health are the Most Responsible Health Care Provider (MRHCP). As MRHCPs the NP is responsible and accountable for directing patient care, facilitating interdisciplinary collaboration, and maintaining accountability for aligning all medical decisions with the patient's values and goals.
In this role you will also:
- Work autonomously to diagnose, order, and interpret diagnostic tests with a strong commitment to clinical excellence and patient safety
- Prescribe pharmaceuticals and perform procedures within your professional scope of practice
- Partner with physicians and interdisciplinary care teams to support seamless, compassionate transitions from hospital to home
- Build trusted relationships with patients and families, ensuring care plans align with their goals, values, and lived experiences to transition patients to home and community.
- Contribute to quality improvement initiatives, research, and innovation that advance transitional health care across the system
Casual Schedule
- We are open to a flexible scheduling model that would allow NPs to share a casual schedule in alignment with the casual scheduling provisions outlined in the NSNU MOA. NPs interested in a casual position must be able to provide weekday and weekend coverage.
About You
You're a compassionate, forward-thinking healthcare professional who thrives in collaborative, patient-centered settings. You believe in treating every person with dignity, leading with professionalism, and creating safe spaces for healing and recovery.
You bring:
- A Master's in Nursing Nurse Practitioner Stream and Certification
- Current registration with the Nova Scotia Nursing and Midwifery Regulator in good standing
- Experience supporting vulnerable or complex populations through admission, care planning, or discharge (an asset)
- A passion for helping patients safely return home and maintain independence
- Fluency in English
