Job description:

The Social Worker - BSW Senior, reporting to the Manager or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals, and consultation to appropriate services and provides on-site and virtual social work services. Education

  • Bachelor of Social Work (BSW)

Experience

  • 2 years SW experience working with issues of capacity, dementia, grief, and loss
  • Two years experience with social work practice in a medical setting

  • Ability to work in a team setting in a postive manner
  • Knowledge in medical terminology
