- Provide comprehensive nutrition assessments, individualized care plans, and follow-up for clients across the lifespan
- Deliver nutrition counselling related to chronic disease management, preventative care, maternal and child health, and general wellness
- Collaborate with physicians, nurse practitioners, nurses, and allied health professionals to support integrated primary care
- Support clients in setting realistic, achievable nutrition goals that align with their health needs and preferences
- Develop and deliver individual and group-based nutrition education sessions as required
- Contribute to health promotion initiatives and community-based programming
- Maintain accurate, timely, and confidential clinical documentation in electronic medical records
- Participate in interdisciplinary team meetings, case conferences, and care planning discussions
- Support quality improvement initiatives and data collection as required
- Adhere to professional standards, privacy legislation, and organizational policies
- Promote culturally safe and respectful care grounded in relationship-building and client empowerment
- Bachelor’s degree in Nutrition or Dietetics
- Registered and in good standing with the College of Dietitians of Ontario
- Experience working in primary care, community health, or clinical settings
- Strong knowledge of chronic disease prevention and management
- Experience working with diverse populations and complex health needs
- Excellent communication, counselling, and interpersonal skills
- Strong organizational skills with the ability to manage a varied caseload
- Proficiency with electronic medical records and standard office software
- Ability to work independently and collaboratively within a team environment
