SALARY RANGE: $26.22 - $33.24BENEFITS:Health and Dental Benefits RRSP 5% matched after 1 year Above ESA standard vacation entitlementFloat Holiday 12 Sick days per year SUMMARY OF THE POSITIONThe Assertive Community Treatment Team (ACTT) is an multidisciplinary team that provides comprehensive, community-based psychiatric treatment, rehabilitation, and support to persons who experience serious and persistent mental illness or concurrent issues that are complex and cause significant functional impairments. This person-centered, recovery-oriented outreach model helps people achieve their goals, improve their health and quality of life, and live as independently as possible
The Social Worker will:· Conduct comprehensive psychosocial and mental status assessments. · Take a lead role in providing intensive treatment, rehabilitation, counselling, coaching, and support based on a personal support plan developed with consumers to help them reach their potential. · Be the primary case manager for particular clients and back up case managers for others. · Develop team activities that provide support to families· Assist consumers with identifying their needs for skill enhancement in their relationships with others.· Assist in linking consumers with community resources and providing consultations to other team members.
HOURS OF WORKThe Social Worker will work a thirty-five (35) hour week, the hours being flexible to the needs of the program. This position participates in a rotating team schedule including in one (1) evening shift per week and one (1) weekend shift every nine (9) weekends. ESSENTIAL QUALIFICATIONS
- Bachelor or Master’s Degree in Social Work from an accredited university.
- Registration with the Ontario College of Social Workers and Social Service Workers.
- Three to five years’ experience in a community mental health setting working with adults recovering from serious mental illness.
- Certification in CPR, non-violent crisis intervention and in suicide risk assessment preferred.
- Knowledge of mental health resources within the Waterloo Region.
- Demonstrated skill in applying the principles of PSR, Recovery and case management.
- Working knowledge of the Mental Health Act and other relevant legislation.
- Specific training in family intervention approaches.
- Experience using a harm reduction approach and motivational interviewing.
- Experience working with individuals who have both mental health challenges and acute housing instability.
- Experience in supporting individual vocational programing and collaboration with community resources.
- Willing to travel extensively within the catchment area and to work evenings and weekends.
- Must have a valid driver’s license, relevant insurance and use of a reliable vehicle.
- Proficient in MS Office Suite, PC Skills, and documentation.
- Satisfactory police vulnerable sector check.
