Provides assessment, intervention, monitoring and follow-up for a broad range of acute and chronic conditions. provides exemplary quality care through the application of advanced clinical and theoretical knowledge and skills. Demonstrates sound clinical reasoning in decision making, planning and implementing care. Utilizes skilled communication with patients, families, healthcare providers, and partners, to support coordinated, compassionate care, including Advance Care Planning and goals of care conversations. Builds partnerships with community and external stakeholders to support care planning and continuity of care. Continuously identifies areas of improvement and provides leadership for quality improvement initiatives. Provides leadership and promotes excellence in patient-centred nursing care through expert practice, education, consultation and research. Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP-Adult or NP- Primary Health Care required. Master's in Nursing/Nurse Practitioner (MN-NP) or Master's in Nursing with successful completion of a post Master's Nurse Practitioner Certificate Program from a recognized school of graduate studies required. 5 years of relevant acute care or post-acute experience in the inpatient setting (e.g. medicine, medical surgical nursing, intensive care, complex continuing care). 1-3 years of primary care experience an asset. Experience with complex wound care assessment and treatment an asset. Additional specialty geriatric education and experience with seniors and frail elderly preferred. Experience in caring for people with mental health and/or geriatric psychological conditions an asset.
