What we are looking for:

As a member of the Adult Diabetes Education Clinic team, you will provide diabetes education to our patients in the inpatient and outpatient clinics in both group and individual settings.

Work schedule:

Monday to Friday. Combination of 8- and 12-hour shifts.

Please note schedules may change due to operational needs.

Compensation:

OPCOM07

Range: $39.22 - $49.95

Responsibilities
  • As a member of our inpatient clinical dietetics team, you will be responsible for the nutritional assessment, care plan implementation and evaluation for a variety of acute and subacute adult patient populations at Markham Stouffville Hospital's Markham, Uxbridge and RCC sites.
  • In this role, you will, in collaboration with the interdisciplinary health care team, be responsible for comprehensive assessment, care planning and nutritional education for patients and families with diabetes.
  • You will practice in an environment where the emphasis is on a high standard of diabetes care, professional practice and patient experience.
  • You will coordinate your services with the other staff in the Clinic, which will require day and evening as well as potential travel to our satellite sites. In addition, you will participate in ongoing quality improvement and promotional activities, committees and the development of standards and policies as it relates to our Diabetes Program.
  • Your strong diabetes knowledge and counselling skills, coupled with well-developed analytical, problem-solving and interpersonal skills, will assist you in the provision of quality patient care.
Requirements
  • Current membership in the College of Dietitians of Ontario, in good standing.
  • Certified Diabetes Educator designation required (or demonstrates strong knowledge in diabetes education & management and willingness to obtain CDE certification).
  • Membership with Dietitians of Canada (DC), preferred.
  • CNSC designation is an asset.
  • Bilingualism in a South Asian dialect is an asset.
  • Certified Pump Trainer is an asset.
  • Demonstrated proficiency in diabetes pharmacotherapy management and education – including insulin and OHA adjustment.
  • A minimum of 2 years' clinical experience in nutrition care support of adult patient population in an acute care setting.
  • Demonstrated commitment to ongoing professional development and evidence-based practice.
  • Excellent assessment, organizational, and interpersonal skills.
  • Demonstrated excellence in verbal and written communication skills.
  • Computer skills are essential.
  • Demonstrated competency with EMR charting systems, CBORD, and associated computer skills.
  • Ability to work independently and as part of an interprofessional team.
  • Candidates must have own transportation and the flexibility to travel between Markham, Uxbridge, RCC and satellite clinics, as necessary.
  • Demonstrated good attendance and performance records with the ability to maintain these same standards.
  • Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with organization-related policies, health and safety legislation and best practices and completing relevant mandatory education as required.
