Palliative Care Program Full Time and Part-Time roles The Palliative Care Program is dedicated to delivering comprehensive home nursing services tailored to meet the unique needs of individuals, families, and groups facing end-of-life circumstances. Our team of skilled nurses is committed to elevating the standard of care for palliative clients, with a primary focus on proficient symptom and pain management. Through our specialized services, we aim to enhance the overall quality of life for individuals receiving palliative care. Our dedicated nurses operate in the North York and Durham region, providing compassionate and personalized care within the comfort of clients' homes. There is a $2000 Sign-On Bonus for full-time nurses hired to work in the Durham region only. The Palliative Care nurses will provide palliative assessments and end-of-life care to patients. Some of the nursing services that you will be providing include wound care, IV therapy, injections, removal of PICC lines, chemo disconnections, and more. Nurses will attain specialized clinical skills in (Patient-Controlled Analgesia (PCA) pumps, pain management, and symptom management). Additionally, doing mental health support and check-in with clients is also required. Hold a valid First Aid or CPR Certification. Preferences are given to candidates who have completed their Fundamentals of Palliative Care, LEAP, and/or CAPCE courses or work on these certificates within 1 year of hire with VHA. Strong clinical practices with the ability to champion evidence-based best practices – particularly in pain and symptom management and titrate medications as required. Demonstrate ability to communicate effectively and problem-solve complex clinical situations and psycho-social circumstances and distress. Able to recognize safety issues, follow safe work practices, and advocate for client care and safety concerns. Able to practice nursing in accordance with client/family-centered care principles, establish an authentic/empathetic professional relationship, and advocate for clients/families. Successful completion of the Physical Readiness Test (PRT) to ensure safe work practices are followed (transfers, bending, lifting, pushing, pulling). A valid Ontario driver's license (G License) and a reliable vehicle for traveling is required.
