To support us on our journey, Sinai Health System is looking for a Social Worker – in patient on General Internal Medicine to support our Social Work Department at our Mount Sinai Hospital Campus.

As an active member of the inter-professional team on a high volume, fast-paced unit you will provide a range of social work services including assessment, crisis intervention, counselling and comprehensive discharge planning with individuals and families. Key components of the role include conducting psychosocial assessments and providing interventions based on a variety of issues such as ageing and dementia, oncology, homelessness, addictions, child protection, mental health and domestic violence as well as linking with community agencies to ensure coordination of appropriate services for patients. As a member of a dynamic social work department you will participate in professional development, teaching and research.

Job Requirements

  • Master's Degree in Social Work
  • Current registration with the Ontario College of Social Workers and Social Service Workers
  • Experience with a range of discharge planning in a hospital setting preferred
  • Experience with consent and capacity preferred
  • Demonstrated ability in individual and family counselling
  • Knowledge of community resources
  • Demonstrated ability to work independently and in a team environment
  • Excellent interpersonal, verbal and written communication skills
  • Demonstrated interest in education and research
  • Demonstrated satisfactory work performance and attendance record
