Position: ACT Initiative Coordinator - 3 Year Contract

Hours: Full Time 40 hours per week. Hours must be flexible to meet the needs of the team, the people receiving support and their families.
Salary: $60,665.64 per annum plus 5% benefits pay plus 4% vacation pay

Overview Of The Role:
Community Living York South is seeking two Achieving Community Transitions (ACT) Initiative Coordinators to lead the implementation of the ACT Initiative over the next three years. This essential program supports individuals aged 15 to 50 with intellectual disabilities across York Region, focusing on enhancing independence and creating pathways to meaningful employment.
The ACT Initiative Coordinator will be responsible for developing person-directed plans that outline actionable steps to help participants achieve their goals related to independence and employment. The role involves providing personalized supports, guidance, and coaching, as well as facilitating the implementation of these plans in collaboration with the participants, partners and employers.
Additionally, the Coordinator will establish and foster strong partnerships with community organizations and businesses to maximize opportunities available to program participants. With the backing of dedicated funding, this role will contribute to Community Living York South's mission of delivering impactful, life-enriching opportunities for individuals throughout York Region.
What You Will Do:
Identify and engage program participants in the ACT Initiative.
Conduct assessments and intake meetings with individuals and families to evaluate needs and assist participants in developing Person-Directed Plans.
Match participants with appropriate partners or develop new partnerships based on their goals and needs, ensuring active support in helping youth achieve their objectives.
Address concerns and provide alternative solutions to support goal achievement, including on-site visits to monitor participants' progress.
Guide individuals in applying for funding opportunities (such as grants and subsidies) and foster partnerships to enable families to independently establish programs or utilize existing resources.
Proactively network, collaborate, maintain, expand and build partnerships with employers, employment services and community organizations to create equitable and gainful opportunities for individuals with intellectual disabilities.
Implement outreach plan to raise awareness about the initiative and recruit participants
Organize bimonthly meetings with collaborative partnership group and ensure timely follow-up on action items.
Foster positive relationships with participants and their families, service providers, employers, and organizations to develop a flexible continuum of services available to program participants.
Act as a liaison between service providers/partners and participants to facilitate effective communication and collaboration.
Fulfill reporting requirements aligned with the agency's strategic plan and funder expectations.
Prepare and submit funding proposals to secure resources for program expansion and sustainability.
Participate in projects and initiatives as directed by the supervisor.
What We Need:
We are seeking values-driven professionals who are passionate about supporting individuals with intellectual and developmental disabilities. The ideal candidate will possess the following:
A Bachelor's degree in Arts, Psychology, Social Work, or a related field; or a diploma in Developmental Services or Social Services, with relevant experience considered an asset
Ontario Secondary School Diploma (minimum requirement)
1-2 years of experience in the developmental services sector preferred
Demonstrated ability to support individuals across a range of ages and abilities
Strong knowledge and application of core competencies () within the developmental services sector
Proven leadership skills with the ability to mentor, model, and guide others
Strong advocacy skills and the ability to foster independence in individuals
Experience in community development and building community capacity
Proficiency in MS Office, Adobe InDesign, and internet research tools
Excellent verbal and written communication, public speaking, and presentation skills
Exceptional interpersonal, problem-solving, and organizational skills
Ability to plan, coordinate, and facilitate workshops and special events
Experience engaging in project-based work
Knowledge of employer engagement and employment services in York Region and surrounding areas
Flexibility to travel throughout York Region
Valid Ontario G Class Driver's License with access to a reliable vehicle
Fluency in a second language such as Cantonese, Mandarin, Hindi, Urdu, Punjabi, Farsi, Korean or French is considered a strong asset
