Health PEI offers a number of Social Worker positions available within the Mental Health and Addictions (MH&A) division and located across the province. Join us at an exciting time as we are implementing new programs, and opening modern, fit-to-purpose facilities for both acute and community care under our comprehensive Redevelopment Project !

A MH&A Social Worker provides collaborative care as a member of a multidisciplinary team including but not limited to Nursing, Psychiatry, Psychology, Recreation Therapy, and Occupational Therapy. Social Workers will provide a range of therapeutic and evidence-based interventions which include assessments, counselling, treatment coordination, case management, group programming, and therapeutic support services to adults, youth, and their family members and/or significant others impacted by difficulties with their mental health, substance use, or behavioural addictions.

Opportunities within acute and transitional care include working with patients and clients with diverse needs in inpatient, day treatment, and community residential settings.

Opportunities in community include the newly established Flexible Assertive Community Treatment Teams, the Provincial Adult ADHD Program, working with adults and children in a community clinic, working with youth and parents as part of a Student Wellbeing Team, and working with Veterans and members of the Armed Forces and RCMP at PEI's Operational Stress Injury Clinic.

Duties will vary according to the specific position, and may include:

  • Providing a wide range of evidence-based screening, assessment, and treatment modalities in consultation with clients/patients.
  • Performing Psycho-social assessments that include considering social determinants of health and identifying risks and coping or mitigation strategies
  • Providing care planning and navigation support, including referrals to appropriate services
  • Collaborating with interdisciplinary team members to coordinate and participate in case conferences and rounds as appropriate, and to promote a team approach care
  • Conducting follow up with individuals as required to ensure they are meeting their goals; provide education and support to understand their condition, triggers and how to manage impulse control, and behaviors
  • Liaising with other programs and community partners to support a comprehensive approach and identify opportunities to expand and improve quality of care.

Qualifications:

  • Bachelor degree in Social work or Master's degree in Clinical Social Work (depending on role)
  • Must be eligible for registration with the PEI Social Work Registration Board; where currently registered, must be a member in good standing.
  • Clinical mental health and substance use experience. (duration of experience specific to role)
  • Working knowledge of the Mental Health Act, the Diagnostic and Statistical Manual for Mental Disorders 5 and other relevant Federal and Provincial statutes and acts.
  • Experience and training in individual, family and group therapy.
  • Good communication skills both orally and in writing.
  • Must be able to provide own transportation with mileage reimbursement as per collective agreement.
  • The successful applicant must have a good previous work and attendance record.

Salary Range: 35.58-44.48 or 39.63-49.55 per hour (Dependent on role)

Bi-weekly Hours: 75.0 hours Bi-weekly / Monday - Friday Days; Evenings and Weekends required for some positions.
