SCHEDULE: Day, Evening, Weekend, On Call, Rotation, 8 and 12-hour shifts. Work schedule may be changed to meet operational needs.

JOB SUMMARY:
Under the direction of the Renal Health Learning Client Program and the department manager, the successful candidate provides advanced clinical expertise in clinical assessment and teaching of patients with end-stage kidney disease. They also provide patients and their families with tailored clinical and theory-based support. They provide leadership and care coordination, in-clinic and remote monitoring, and telephone support for patients at home. They teach patients how to do their hemodialysis treatment semi-autonomously or at home. The successful candidate facilitates and coordinates transitions home, working closely with multidisciplinary team members. They help develop and implement care plans based on best practices, evidence and current trends. They take part in continuing education activities to keep their nephrology knowledge and skills up to date. The incumbent may be called upon to perform other related duties as required by the department.
REQUIREMENTS:
- Registered with the College of Nurses of New Brunswick;
- Bachelor of science in nursing;
- Minimum of two years' clinical experience in hemodialysis within the last five years;
- Nephrology certification from the Canadian Nurses Association (CNA), to be obtained within agreed timeline;
- Valid Basic Life Support (BLS) certificate and maintenance of certification;
- Proficiency with the Microsoft 365 suite;
- Knowledge of software programs currently used within the Network;
- Rigorous clinical judgment and strong clinical analysis skills;
- Knowledge of best practices in nephrology;
- Proven teaching skills;
- Organizational skills, ability to adapt to change and interest in professional development;
- Proven communication and interpersonal skills;
- Valid driver's licence and willingness to use a personal vehicle for work purposes;
- Willingness to travel within the Network's service area.
- Ability to work independently and as part of a team;
- Physical ability to perform the assigned work;
- Good work history (performance and attendance);
- Adherence to professional ethical principles, the Network's management philosophy, and organizational values;
