Horizon Health Network in beautiful New Brunswick is looking for a Regional Director of Population Health to join our team!

Horizon Health Network (Horizon) is seeking a strategic, motivated, and agile individual to join our team as a Regional Director of Population Health. The ideal candidate will be a senior expert on population health, providing expertise and strategic support regionally on population health initiatives, system improvements and identifying and addressing health disparities within a large organization.

Who you are

You hold a master's degree in health or social science discipline. A concentration of study or advanced degree on population health and/or social determinants of health is considered an asset for this position. You have the ability to work with minimal supervision and possess strong organizational and time management skills. You have 7 to 10 years progressive leadership experience with experience in human resource and financial management, problem solving and critical thinking. You have additional training in project management and organizational change/improvement and trauma informed care (an asset). Using a Two-Eyed seeing approach, you foster an environment that supports traditional Indigenous ways of knowing & healing practices with evidenced based quality care. You also possess additional experience leading collaborative initiatives in a clinical setting (asset). You have demonstrated program and project leadership skills, including expertise in planning, implementation, and evaluation of programs.

What you will do

  • Develop and execute strategic plans in addition to develop, implement, monitor, and evaluate operational plans related to Population Health services in alignment with Horizon's Strategic Plan.
  • Provide strategic leadership to Horizon Health Network in the adoption of evidence-informed approaches to improve population health and effect system transformation.
  • Identify and support research, best practices and projects that align with the mandate and strategic direction of the department and Horizon Health.
  • Support the facilitation of a Two-Eyed Seeing approach to wellbeing and care of Indigenous clients and building these systems within Horizon Health Network.
  • Serve as a strategic business partner, advisor, and leader to influence all levels of the organization by providing and executing responsive solutions.
  • Work closely with Department of Health and other government departments and external stakeholders to enhance policy and strategically support the delivery of population health priorities and assure the modernization and effective delivery of population health services.
  • Leads a regional approach to evidenced based practices in community engagement in collaboration with public health, primary care, addictions and mental health partners as well as other stakeholders in Horizon.
  • Supervise multiple operational population health teams delivering services within Horizon and communities within New Brunswick.
  • Oversee, monitor, and evaluate the quality and safety of Population Health services.
  • Prepare, manage, monitor, and report on budgets and ensure fiscal responsibility within all departments.
  • Develop, review, revise, monitor and report on performance indicators.
  • Oversee the processes that ensure adherence with professional practice standards, policies, and procedures.
  • Oversee human resource planning, skill optimization & utilization, productivity & capacity enhancements and assist with recruitment and retention activities.
  • Build and maintain relationships with physicians and internal and external stakeholders, including integration of population health approaches within the overall Community portfolio and within the larger Horizon system.
  • Participate/coordinate quality improvement activities and risk management processes to improve quality of care with a specific focus on taking a regional approach to improvements & increased accountability, accessibility, productivity, capacity, responsiveness.
  • Provide leadership and oversight of standardization of policies and procedures across all zones in collaboration with local Directors and Managers of Population Health and Public Health.
  • Address strategic priorities related to health equity and the social determinants of health by building capacity in the organization is a focus of this portfolio.
  • Travel throughout the region/province as required.
