Job Summary:
This is a professional support position which provides administrative and supportive services to case managers in the daily provision and coordination of activities necessary for effective client service delivery and case management within the Community Support Program. Responsibilities of this position include direct client/public contact, analytical and administrative functions. This Social Assistance Worker is part of an interdisciplinary team collaborating with case managers, home support coordinators, financial assessors and external agencies to coordinate care for clients. Will be working with in the new digital Home Support Solution, performing data entry, sending digital referrals electronically and monitoring referral acceptance. Need to ensure client clinical information is current, monitoring individualized support plans to ensure accuracy within the Home Support Solution. Provides ongoing feedback to case managers and home support coordinators regarding provision of services.

Alertness and concentration required while frequently completing repetitive tasks and constant auditory/visual concentration.
Qualifications:
Graduation from a recognized/approved bachelor?s degree in social sciences/Humanities or Business is required.
Knowledge of community support agencies, programs and eligibility requirements. Knowledge of complex health concerns and/or mental health issues.
Effective time management, communication, organization and strong interpersonal skills are essential.
Applicants must be prepared to demonstrate an ability to be self-directed and exercise independent clinical judgement in a community-based environment with limited supervision.
Demonstrated strong oral and written communication skills.
