Job SummaryThe Community Health Nurse I is a member of an interdisciplinary team providing services to the Personal Care Home (PCH) Program and residents. The Community Health Nurse completes regular visits to assigned personal care homes to assess their compliance with the Personal Care Home Program Provincial Standards; liaises with other team members, Eastern Health professionals and Service Newfoundland regarding these standards; works with personal care home operators to achieve compliance and provides appropriate residential care; provides educational in-services to PCH operators/staff, including but not limited to medication administration, documentation in PCH, incident reporting, and falls prevention; develops other appropriate educational in-services in consultation with the multidisciplinary team as required; investigates and responds to complaints related to personal care homes. Also provides case management services for residents of personal care homes including assessments, reassessment, transfer coordination, access to and authorization of eligible services. Work is performed in accordance with professional nursing standards and the policies of Eastern Health. Evening and/or weekend work may be required.

Occasionally lifts objects Regularly performs counselling sessions with clients and or family members with a requirement to sit for prolonged periods.
May require constant sitting for meetings, documentation, or computer research.
Constantly uses fine finger/precision, auditory, sensory, and visual concentration to provide and conduct assessments, observe/monitor clients, provide feedback to clients and other health care providers and complete documentation.
A high level of attentiveness and vigilance is required when working alone and during counselling sessions to ensure safety or detect impending danger/harm to self or client.

Job QualificationsGraduation from an approved/recognized post-secondary nursing education program is required.*
Current and continuing practicing license with the College of Registered Nurses of Newfoundland and Labrador (CRNNL) and BLS Certification is required.
Experience working in Community Health nursing is an asset.
Demonstrated ability to be self-motivated and able to work both collaboratively and with a high degree of independence and initiative.
A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
A satisfactory record of work performance is required.

* Applicants who do not possess a Bachelor Degree in Nursing must be willing to complete an approved/recognized post diploma/certificate program in Public Health/Community Health at the first available opportunity as determined by the employer. The successful applicant must successfully complete the post diploma/certificate program within the program?s designated timeframe and will start work in the position upon successful completion. Funding to complete the program will be provided by NLHS.
