POSITION: Resident Care Navigator
LOCATION: Yarmouth, NS
TYPE: Full-Time
Villa Saint-Joseph du Lac, located in Yarmouth NS, is home to 79 long-term care Residents. We are a provincially funded, non-profit organization that provides 24/7 care. Villa Saint-Joseph has a strong interprofessional team and a new facility (opened in 2015) which allows us to promote a safe, clean, home-like environment for our Resident and care partners.
Responsibilities (including but not limited to):
- We are seeking a compassionate and dedicated Resident Care Navigator. As a Registered Nurse in this role, you will be responsible for ensuring high-quality care and support for our residents while fostering a positive and collaborative environment for families, staff, and external partners.
- This role requires strong leadership, exceptional communication skills, and a deep understanding of resident-centered care practices.
- This role includes facilitating regular Family Council meetings and participating in Resident Council meetings to promote open communication and engagement between families, residents, and the facility.
- The Navigator advocates for residents' well-being and rights, ensuring their care plans reflect individual preferences, values, and goals, while collaborating closely with the healthcare team to ensure the highest standard of care.
- Effective communication, problem resolution, and teamwork are essential, as the Navigator resolves issues related to care, environment, and policies.
- They also manage investigations related to the Protection for Persons in Care Act and ensure compliance with best practices.
- The Navigator uses the InterRAI system to track resident quality indicators, ensure regulatory compliance, and provide staff training on data collection and reporting.
- Additionally, they actively participate in quality improvement initiatives, including facility surveys to identify areas for improvement, and serve as a liaison with external organizations to connect residents and families with necessary resources and support.
- This is a Monday-Friday position but there can be flexibility with work hours.
Qualifications:
- Currently registered with the College of Registered Nurses of Nova Scotia
- Minimum of three years of progressive nursing experience in which leadership, administrative and interpersonal skills have been demonstrated
- Past Long Term Care experience is an asset
- Must have excellent communication skills both verbal and written
- Must have excellent conflict management skills
- May require flexibility with work hours at times
- InterRAI experience is an asset
