JOB SUMMARY:The incumbent will report to the patient trajectory manager for Zone 1B. They will be responsible for managing the Dalhousie hemodialysis satellite unit, including renal protection, pre-transplant and post-transplant clinics, as well as nephrology patient trajectory in zones 4, 5 and 6. They will have a regional responsibility for organizing, coordinating, planning and evaluating activities related to nephrology patient trajectory for the renal protection and transplant clinics in zones 4, 5 and 6. They will be responsible for the local management of nursing staff, as well as the physical and material resources in Zone 5 for the hemodialysis unit, in collaboration with the manager of the hemodialysis unit at the Chaleur Regional Hospital. The incumbent will also ensure quality of care and patient safety, while maintaining effective communication between staff, patients, the multidisciplinary team and the community. One of the key aspects of this role will be to coordinate patient flow through the renal care pathway, fostering optimal patient management to maintain autonomy, preserve renal function, promote discharge and avoid hospitalization. This involves assisting patients in choosing autonomous dialysis modalities when appropriate, and referring them to the most appropriate care to effectively manage the disease. The aim is to offer a seamless care pathway that prioritizes patients' individual needs, promotes their autonomy, and upholds their quality of life. The incumbent will also support the program manager in administrative management and facilitate staff autonomy, decision-making and change necessary to meet patient needs and improve the quality of services offered.

REQUIREMENTS:

  • Member of the Nurses Association of New Brunswick (NANB);
  • Bachelor of Nursing;
  • Basic life support (BLS) certificate and maintenance of certification;
  • Minimum of five (5) years' nursing experience in the past eight (8) years;
  • Minimum of two (2)years' recent experience in health care management in a hospital setting considered an asset;
  • Recent experience in nephrology considered an asset;
  • Strong interest in expanding their knowledge and follow training;
  • Proven leadership skills;
  • Proven ability to solve problems, make decisions and resolve conflicts;
  • Proven ability to organize and manage their work and perform self-directed work;
  • Proven communication and interpersonal skills;
  • Proven ability to work in a changing work environment;
  • Ability to work independently and as part of a team;
  • Physical ability to perform assigned work;
  • Good work history (performance and attendance);
  • Adherence to professional ethics principles, Vitalité Health Network's management philosophy and organizational values;
  • Adherence to Vitalité Health Network's confidentiality rules.
