Horizon Health Network (Horizon) in beautiful New Brunswick is seeking a dynamic, collaborative, and strategic leader to join our team as the Executive Director of Primary Care.

About the Role

Reporting to the Vice-President, Community, and working closely with the Primary Care Physician Lead, the Executive Director of Primary Care will provide strategic and operational leadership for Horizon's Primary Care Program. This includes oversight of clinic?based and community?based services delivered across Horizon's 46 community health centres and new family health teams.

The successful candidate will be a system?minded leader with a strong understanding of primary health care models, population health needs, and team?based service delivery. They will be committed to strengthening patient access, improving continuity and coordination of care, and supporting primary care providers and interdisciplinary teams to deliver high?quality, person?centred services.

Who You Are

You are an accomplished healthcare leader with at least eight years of progressive administrative experience, ideally within primary care, community health, or other frontline care environments. You are passionate about building cohesive, high?performing teams that improve the quality, access, efficiency, and delivery of primary care.

You excel at designing and leading complex programs, supporting interdisciplinary teams, and developing strong partnerships with providers, community organizations, and system stakeholders. You foster a culture of accountability, engagement, and continuous improvement—driven by strong budgeting experience, project and change management expertise, and a deep understanding of healthcare regulations and service standards.

Education Required:
Master's degree in business or healthcare management, or equivalent training and experience within a clinical profession.

Years of Experience Required:
8–10 years of progressive administrative and management experience in the healthcare sector.

Additional Training:
LEAN Six Sigma Black Belt, process improvement training, and project management experience are considered strong assets.

What You Will Do

As the Executive Director of Primary Care, you will play a pivotal role in shaping the future of primary healthcare delivery in New Brunswick. Your responsibilities will include:

Strategic Planning & Development

Developing a forward?looking strategic plan and annual operational plan for the Primary Care Program aligned with Horizon's corporate strategy and population needs.

Designing and implementing initiatives that improve access, team?based practice, and continuity of care.

Establishing and monitoring objectives and key performance indicators (KPIs) that reflect Horizon's Mission, Vision, and Values.

Collaboration & Communication

Working collaboratively with physicians, nurse practitioners, clinic managers, and community partners to enhance primary care access and integration.

Building strong relationships with regional and provincial colleagues to support policy and system improvements.

Maintaining effective communication with the dyad leadership team, other programs, and all staff to foster consistency, alignment, and engagement.

Program Delivery & Evaluation

Monitoring program performance, budget variances, and resource utilization to support responsible stewardship.

Implementing and evaluating quality improvement initiatives that enhance patient outcomes, safety, and service efficiency.

Ensuring accountability for organizational priorities and standards within the Primary Care Program.

Promoting a positive, supportive work culture that values professional development and team?based care.

Staying informed on best practices and emerging models of primary care locally, nationally, and internationally.

Patient and Family Engagement

Establishing mechanisms to hear and act on patient and family feedback to ensure services remain person?centred and responsive.

Measuring and evaluating the effectiveness of primary care delivery and patient outcomes to drive continuous improvement.

Leadership & Reporting

Leading the Primary Care Program in accordance with principles of organizational design and regulatory standards.

Promoting a culture of innovation, continuous improvement, and clinical excellence.

Preparing and presenting reports and recommendations to senior leadership and board?level committees as required.
