This innovative facility provides specialized, person-centered care for individuals who no longer require hospital-level treatment but need additional support to safely return home or reintegrate into their community. The Nutrition Assistant is integral in supporting the goal of returning to home/community by supporting the Registered Dietitian with various aspects of nutrition care, including patient interviews to gather diet history and allergies/sensitivities; preparing educational materials; researching product ingredients; and by providing administrative support for the culinary team, including data entry into the electronic menu software, training patient and staff with the electronic menu system.

About the Opportunity

  • Conducts patient interviews and nutrition screens to gather information about dietary needs, allergies/sensitivities and ensures patients' weights are entered into the healthcare record.
  • Enters admission documentation diet orders into the Electronic Healthcare Record (EHR) and communicates with the Culinary Team and Dietitian.
  • Collaborate with patient/family, Dietitian, and culinary team to ensure the menu selection options meets the patient's dietary needs.
  • Gathers detailed product information to support patient allergies/sensitivities/ dietary needs.
  • Provides basic nutrition and healthy eating education to support healthcare outcomes.
  • Inputs menu information into the electronic menu software, including therapeutic diets, texture modified diets, specialty diets, allergies, and sensitivities
  • Provides staff education and training to use the electronic menu system
  • Develop and implement health promotion activities and resource materials for patient education related to healthy eating habits.

About You

In addition to placing high value on continuous improvement, collaboration and accountability, you bring:

  • Undergraduate degree in Nutrition
  • Knowledge of therapeutic diets, texture modified diets, specialty diets, basic nutrition, food allergies and sensitivities
  • Food safety certification
  • Minimum of one-year experience working in nutrition within an acute care setting
  • Strong Computer skills and data entry experience
  • Knowledge of electronic menu systems in a healthcare setting
  • Excellent communication, teaching, time management and organizational skills
