About the Opportunity
The transitional care model bridges hospital and home, supporting patients who need extra time and care to return safely to their communities. Rooted in dignity, safety, and partnership, this model ensures each patient's journey is guided by evidence-based practice and meaningful human connection.
The Nurse Practitioner works collaboratively to create care plans that help patients achieve their goal of going home safely and confidently. In this role you will also:
- Work autonomously to diagnose, order, and interpret diagnostic tests with a strong commitment to clinical excellence and patient safety
- Prescribe pharmaceuticals and perform procedures within your scope of practice
- Partner with physicians and interdisciplinary care teams to support seamless, compassionate transitions from hospital to home
- Build trusted relationships with patients and families, ensuring care plans align with their goals, values, and lived experiences
- Contribute to quality improvement initiatives and innovation that advance transitional health care across the system
About You
You're a compassionate, forward-thinking healthcare professional who thrives in collaborative, patient-centered settings. You believe in treating every person with dignity, leading with professionalism, and creating safe spaces for healing and recovery.
You bring:
- A Master's degree in Nursing with Nurse Practitioner Certification
- Current registration with the Nova Scotia College of Nursing in good standing
- Experience supporting vulnerable or complex populations through admission, care planning, or discharge (an asset)
- A passion for helping patients safely return home and maintain independence
- Fluency in English
