JOB SUMMARY:

The Indigenous Clinical Services Worker will act as a resource and point of contact for both Indigenous Clients and their families, health care providers, and, when appropriate, community members, to ensure the provision of care is culturally safe and client centred. In collaboration with patients and their families, health care providers, and community resources, the Indigenous clinical patient navigator will navigate the health care system by facilitating access to services and assisting with discharge planning. Using a Two-Eyed Seeing approach, the role promotes an environment that supports traditional Indigenous ways of knowing and healing practices alongside high-quality, evidence-based care. The objective of this position is to ensure that Indigenous patients have access to appropriate care from safe sources and to strengthen patients' capacity and connections, so they are stable and well supported.

REQUIREMENTS:

  • Degree in a health- or social-science-related discipline, or equivalent experience working in Indigenous health programs;
  • Experience working within the health care system is an asset;
  • Minimum of one (1) year of recent, related experience in Indigenous health care;
  • Knowledge of the Mi'kmaq language is an asset;
  • Proficiency with Microsoft 365;
  • Demonstrated knowledge of the Two-Eyed Seeing approach and Indigenous ways of knowing;
  • Demonstrated understanding of Indigenous healing practices and person-centred care;
  • Working knowledge of Indigenous Health programming at the systems, community, provincial and federal level;
  • Demonstrated knowledge of the demographics, languages, cultures, customs, and traditions of local Indigenous peoples;
  • Demonstrated understanding of cultural safety and competencies in health care;
  • Demonstrated understanding of Canadian colonization, systemic racism, and their impact on the current health and well-being of Indigenous peoples;
  • Ability to build trusting relationships;
  • Ability to facilitate or make referrals as needed;
  • Must be of Indigenous descent and able to demonstrate their connection to their community and culture via a testimonial from their Band leadership (Chief and Council) or individual who is recognized by their community as a cultural leader (Elder, etc.);
  • Ability to work independently and as a team member;
  • Physically able to perform assigned duties;
  • Good previous work history (performance and attendance);
  • Adherence to professional ethics principles, to the Network management philosophy and organizational values;
  • Compliance with the rules of confidentiality established by the Vitalité Health Network.
