Definition of Work:
Reporting to the Regional Director of Patient Flow and Site Operations CNRHC & JPMRHC, this position will offer support to acute care social workers at both CNHRC and JPMRHC. These interventions may include, but not limited to, locating and arranging transportation, meal vouchers, oxygen and accommodations; assisting in the completion of applications for funding sources to secure services such as home supports, medical equipment, income support, disability pensions and cancer support; completing financial assessments for medical discharge waivers and liaison with community and government resources on behalf of patients. This work involves patient and family interviews and consultations with a variety of agencies and staff, in person and by telephone. The Social Service Worker provides ongoing feedback to referral sources, social workers and other members of the health care team through consultation and attendance at interdisciplinary rounds as required. Written correspondence and documentation on client?s health records, as well as the completion of statistics and workload measurement, is also required. This position will be responsible for obtaining and coordinating current resource information for clients, families and staff. The successful candidate must demonstrate an awareness of and be responsible for promoting and supporting person-and family-centered care, demonstrate a commitment to safety and partner with all team members to improve worker and patient safety by integrating safety into work practices, identifying risks and implementing improvements.

Required Qualifications:
? The successful completion of a minimum 2-year diploma in a social science from a recognized program.
? A minimum of 2 years? experience within the last 5 years in a healthcare setting.
? Demonstrated ability to work effectively with the public.
? Demonstrated skills in communication, written and oral.
? Demonstrated organizational skills.
? Ability to follow guidelines and processes according to NLHS policies and procedures.
? Consistent demonstration of NL Health Service?s corporate values; I CARE- Innovation, Compassion, Accountability, Respect and Excellence.
? A satisfactory record of work performance and attendance is required for this position.
? A satisfactory certificate of conduct from the RCMP or local police authority.
