Partners for Youth is a provincial not for profit organization and we are seeking a fulltime Registered Social Worker to co-lead our PFY Connect program which is a rapid access youth mental health treatment program.
PFY Connect supports timely rapid access to appropriate counselling therapy for youth ages 14 to 24. Through a simple process, a youth in need of support will make an initial connection to our designated staff team, which includes a bilingual registered social worker.
Referrals can be made to this program by:
• Self -referral,
• Any community member,
• Family,
• School, guidance staff, child and youth team,
• Friend or peer, and
• Professional support.
The youth will then be assessed though a trauma informed intake process to ensure the appropriate counselling need is met and then PFY staff will reach out on behalf of the consenting youth and/or guardian to a licensed counselling therapist, of their choice, and set up services and supports. The goal of this program is to have a youth connected to a licensed counsellor of their choice within 7 days - which we have been consistently successful in achieving.
The project social workers are regulated and registered health professionals who work with youth-individuals, families, partners and communities-to improve the individual and collective well-being of youth in the program. They often work in interdisciplinary teams to provide services, share resources, improve conditions and create opportunities for growth, recovery and personal development.
Within PFY Connect our social workers respond to social issues and address barriers and injustice within society and organizations. They advocate for social justice, human rights and fair access to health and social services.
The project social workers may assess and address the needs of youth, families and groups, advocating for youth when needed. In this role, they work to build community awareness, identify and develop leaders, create strategic alliances, foster collaboration and build community capacity.
The PFY Connect social workers (in both official languages):
Provide emergency counselling to youth and families, directing them to crisis supports as needed.
Act as a clinical support and community service navigator for youth.
Remove barriers to accessing care and treatment as necessary.
Develop and conduct youth and family intake interviews and referrals.
Perform needs assessments and determine involvement for community partners with youth voice at the center of the decisions.
Plan, develop, report on and evaluate the PFY Connect program as directed.
Engage with a vast number of community partners, specifically a developed network of over 100 licensed counselling therapists across New Brunswick and beyond.
Analyze PFY Connect program policies and processes always with the intent of improvement.
Prepare and maintain assessments and statistical reports for our partners.
Contribute to research initiatives as needed.
Advocate on behalf of youth, families and communities to enact change at the structural level.
